儿童腰椎穿刺术护理管理的循证实践

摘要 目的:基于循证方法改进腰椎穿刺术后患儿的护理管理,以期改善医护人员的知识掌握度以及患儿术后舒适度。方法:遵循澳大利亚Joanna Briggs Institute(JBI)临床证据实践应用模式,以基于证据的持续质量改进模式为理论框架,按照证据获取、现状审查、证据引入和效果评价4个阶段将循证实践应用于腰椎穿刺术患儿,比较循证实践前后医护人员相关知识水平、审查指标执行率以及患儿腰椎穿刺术后6 h情况。结果:循证实践后,医护人员相关知识水平提高,审查指标执行率提高,差异均有统计学意义(P<0.05)。结论:循证实践能够提高医护人员相关专业水平及审查指标执行率,但对于腰椎穿刺术后去枕平卧时间及不舒适等结局的发生率仍有待进一步探索。 Objective:To improve the nursing management of children after lumbar puncture based on an evidence⁃based approach,with the aim of improving the knowledge of healthcare professionals as well as the postoperative comfort of children.Methods:The Joanna Briggs Institute(JBI)Clinical evidence practice application model in Australia was followed.The evidence⁃based continuous quality improvement model was used as the theoretical framework.Evidence⁃based practice was applied to children undergoing lumbar puncture according to the 4 stages of evidence acquisition,status review,evidence introduction,and effect evaluation.The level of relevant knowledge of healthcare personnel,the implementation rate of review indicators,and the situation of children 6 h after lumbar puncture were compared before and after the evidence⁃based practice.Results:After evidence⁃based practice,the level of relevant knowledge of medical and nursing staff increased,and the implementation rate of review indicators increased,and the differences were statistically significant(P<0.05).Conclusion:Evidence⁃based practice can improve the level of healthcare personnel's relevant specialties and the implementation rate of review indicators.However,the incidence of outcomes such as de⁃pillowed lying time and discomfort after lumbar puncture should be further explored.
