摘要
目的:分析痛风方联合非甾体抗炎药治疗急性痛风性关节炎的效果。方法:收集2021年10月—2023年12月在北京市鼓楼中医医院进行治疗的急性痛风性关节炎患者122例,其中使用非甾体抗炎药(双氯芬酸钠)的61例为对照组,使用非甾体抗炎药+痛风方的61例为痛风方组,回顾性分析两组患者的临床资料,比较两组患者治疗前后尿酸水平、疼痛情况、血液流变学指标、炎性因子水平、临床疗效,记录给药期间过敏反应、消化道不适、肝肾功能异常等不良反应。结果:治疗后痛风方组血尿酸水平及疼痛视觉模拟法评分显著低于对照组(P<0.01);治疗后全血黏度、血浆黏度、血沉水平,痛风方组显著低于对照组(P<0.05,P<0.01);治疗后血清白介素-1β、血清白介素-8水平比较,痛风方组显著低于对照组(P<0.01);总有效率对比,痛风方组显著高于对照组(P<0.05);不良反应总发生率对比,痛风方组与对照组无显著差异(P>0.05)。结论:痛风方联合非甾体抗炎药治疗急性痛风性关节炎可降低患者血尿酸水平,缓解疼痛,并可改善患者血液流变学,抑制炎症反应,且联合治疗增效但不增毒,安全性高。
Objective:To analyze the efficacy of Gout Formula combined with nonsteroidal anti-inflammatory drugs(NSAIDs)in treating acute gouty arthritis.Methods:A total of 122 patients with acute gouty arthritis treated at Beijing Gulou Hospital of Traditional Chinese Medicine from October 2021 to December 2023 were included.Among them,61 patients treated with NSAIDs(diclofenac sodium)were assigned to the control group,while 61 patients treated with NSAIDs plus Gout Formula were assigned to the Gout Formula group.Clinical data of the two groups were retrospectively analyzed to compare serum uric acid levels,pain intensity,hemorheological parameters,inflammatory factor levels,and clinical efficacy before and after treatment.Adverse reactions such as allergic reactions,gastrointestinal discomfort,and abnormal liver or kidney function during treatment were recorded.Results:After treatment,the Gout Formula group showed significantly lower serum uric acid levels and visual analog scale(VAS)pain scores than the control group(P<0.01).Whole blood viscosity,plasma viscosity,and erythrocyte sedimentation rate in the Gout Formula group were significantly lower than those in the control group after treatment(P<0.05,P<0.01).Serum interleukin-1βand interleukin-8 levels in the Gout Formula group were significantly lower than those in the control group after treatment(P<0.01).The total effective rate was significantly higher in the Gout Formula group than in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:Gout Formula combined with NSAIDs can reduce serum uric acid levels,alleviate pain,improve hemorheology,and suppress inflammatory responses in patients with acute gouty arthritis.The combined treatment enhances efficacy without increasing toxicity and demonstrates high safety.
