摘要
目的探讨丙泊酚复合舒芬太尼在腹腔镜下子宫全切术(TLH)麻醉中的应用效果。方法84例接受TLH治疗的患者,根据随机数字表法分为研究组(42例)与对照组(42例)。研究组患者采用丙泊酚与舒芬太尼复合麻醉,对照组患者采用丙泊酚与芬太尼复合麻醉。比较两组患者不同时间点的血流动力学指标(平均动脉压与心率)、睁眼时间、自主呼吸恢复时间以及麻醉不良反应发生率。结果两组麻醉前平均动脉压、心率比较并无差异性(P>0.05);研究组气管插管时、气腹后5 min、手术结束时的平均动脉压分别为(12.20±1.32)、(12.08±1.32)、(11.39±1.08)kPa,心率分别为(82.65±5.56)、(80.65±3.85)、(79.50±3.56)次/min,低于对照组的(13.16±1.08)、(13.46±1.40)、(12.85±1.22)kPa及(86.00±5.02)、(85.65±4.00)、(83.56±3.02)次/min,结果差异显著(P<0.05)。研究组的睁眼时间(8.50±2.02)min、自主呼吸恢复时间(5.12±1.30)min较对照组的(13.02±2.56)、(7.39±1.53)min更短,结果差异显著(P<0.05)。两组麻醉不良反应发生率比较并无差异性(P>0.05)。结论丙泊酚复合舒芬太尼在TLH中具有较为理想的麻醉效果,能够维持患者血流动力学的稳定性,促进术后苏醒与自主呼吸恢复,安全性较佳,具有应用价值。
Objective To explore the application effect of propofol combined with sufentanil in anesthesia for total laparoscopic hysterectomy(TLH).Methods 84 patients receiving TLH were divided into study group(42 cases)and control group(42 cases)according to random numerical table method.Patients in the study group were given combined anesthesia with propofol and sufentanil,while patients in the control group were given combined anesthesia with propofol and fentanyl.Comparison of hemodynamic indexes(mean arterial pressure and heart rate)at different time points,eye-opening time,recovery time of spontaneous breath and the incidence of adverse reactions to anesthesia between the two groups.Results There was no significant difference in mean arterial pressures and heart rates between the two groups before anesthesia(P>0.05).At the time of tracheal intubation,5 min after pneumoperitoneum and at the end of surgery,the mean arterial pressure in the study group were(12.20±1.32),(12.08±1.32)and(11.39±1.08)kPa,and the heart rate were(82.65±5.56),(80.65±3.85)and(79.50±3.56)beats/min,which were lower than(13.16±1.08),(13.46±1.40),(12.85±1.22)kPa and(86.00±5.02),(85.65±4.00),(83.56±3.02)beats/min in the control group,and the differences were significant(P<0.05).The eye-opening time and recovery time of spontaneous breath of the study group were(8.50±2.02)and(5.12±1.30)min,which were shorter than(13.02±2.56)and(7.39±1.53)min of the control group,and the differences were significant(P<0.05).There was no difference in the incidence of adverse reactions to anesthesia between the two groups(P>0.05).Conclusion Propofol combined with sufentanil has a more ideal anesthesia effect in TLH,which can maintain the hemodynamic stability of patients,promote postoperative recovery and spontaneous breath recovery,and has good safety and application value.
