摘要
文章基于火热灼伤津血导致胞宫不能受血安胎的不孕基本病机,提出阳明中风病传血痹可导致虚劳不孕、伤精不孕、干血不孕、结积不孕、燥瘕不孕。虚劳不孕为阳明血痹日久,营血亏虚所致,可用甘药养血润燥、凉血除痹;伤精不孕为邪热化燥伤血,伤及阴精所致,宜用金石药涩精除积;干血不孕为阴虚内热熏蒸,津血暗灼,停而成瘀所致,宜用虫类药搜剔脉络;结积不孕为邪热煎熬营血成瘀,瘀久不消所致,需配伍苦寒、咸寒之品攻癥除积;燥瘕不孕为寒邪入里,寒热邪气与干血胶结于下焦血室所致,需在苦寒、咸寒的基础上配伍辛温之品以散寒通络。从阳明中风病传血痹入手,对中医辨证治疗不孕具有一定的启发和临床意义。
Based on the pathogenesis of infertility,the fire pathogen hurting blood to damage the uterine circulation(blood arthralgia),this paper reviews Yangming apoplexy progressing blood arthralgia and leading to the infertility of asthenia,shangjing,dry blood,jieji and zaojia.Infertility of asthenia is caused by prolonged Yangming apoplexy,leading to depletion of nutritive blood.It can be treated with sweet herbs to nourish blood,moisten dryness,cool the blood,and eliminate stasis.Infertility of shangjing results from pathogenic heat transforming into dryness,damaging the blood and injuring the essence.This requires the use of mineral-based medicines to astringe essence and eliminate accumulations.Infertility of dry blood arises from internal heat due to yin deficiency,which scorches the fluids and blood,leading to stagnation and blood stasis.It is best treated with insect-based medicines to dredge and clear the meridians.Infertility of jieji is caused by pathogenic heat consuming the nutritive blood,forming blood stasis that persists without resolution.It requires the combination of bitter-cold and salty-cold herbs to attack the obstruction and eliminate accumulations.Lastly,infertility of zaojia is caused by cold pathogens entering the interior,where the interplay of cold and heat,combined with dry blood,forms adhesions in the blood chamber of the lower jiao.This condition necessitates the addition of acrid-warm herbs to bitter-cold and salty-cold medicines to dispel cold and unblock the meridians.Therefore,starting with Yangming apoplexy progressing blood arthralgia,it has certain enlightening and clinical significance for traditional Chinese medicine syndrome differentiation treatment of infertility.
