摘要
目的:探讨尿转铁蛋白(UTRF)、胱抑素C(CysC)及肾损伤分子(Kim-1)联合检测对2型糖尿病患者肾损伤的诊断效能。方法:选取2018年2月至2021年3月于本院内分泌科就诊的158例2型糖尿病患者,根据是否存在肾损伤分为肾损伤组(n=98)和非肾损伤组(n=60)。检测患者血液中CysC及尿液中UTRF、Kim-1含量,Pearson相关性分析方法分析UTRF、CysC与Kim-1水平的相关性,绘制受试者工作特征(ROC)曲线,评估UTRF、CysC及Kim-1联合检测对2型糖尿病肾损伤的诊断价值。结果:肾损伤组患者UTRF、CysC及Kim-1含量水平均明显高于非肾损伤组(P<0.05)。Pearson相关性分析表明UTRF、CysC与Kim-1水平呈正相关关系(P<0.05)。UTRF、CysC、Kim-1及三者联合诊断2型糖尿病肾损伤的ROC曲线下面积(AUC)分别为0.800、0.714、0.816、0.908,灵敏度分别为0.732、0.716、0.803、0.876,特异度分别为0.815、0.821、0.825、0.882。UTRF、CysC及Kim-1联合检测AUC、灵敏度和特异度均高于单一指标检测。结论:UTRF、CysC及Kim-1联合检测可提高2型糖尿病肾损伤的诊断效能。
Objective: To explore the diagnostic efficacy of the combined detection of urinary transferrin(UTRF), cystatin C(CysC) and kidney injury molecule(Kim-1) on kidney injury in type 2 diabetic mellitus patients. Methods: One hundred and fifty-eight patients with type 2 diabetes mellitus who attended our endocrinology department from February 2018 to March 2021 were selected and divided into kidney injury group(n=98) and non-kidney injury group(n=60) according to the presence or absence of kidney injury. CysC in the blood and UTRF and Kim-1 in the urine of the patients were detected, Pearson correlation was performed to examine the correlation between UTRF, CysC and Kim-1 levels, and receiver operating characteristic(ROC) curves were drawn to assess the diagnostic value of combined UTRF, CysC and Kim-1 detection for kidney injury in type 2 diabetes mellitus. Results: The levels of UTRF, CysC and Kim-1 were significantly higher in the kidney injury group than in the non-kidney injury group(P<0.05). Pearson correlation analysis showed a positive correlation between UTRF, CysC and Kim-1 levels(P<0.05). The area under the ROC curve(AUC) of detections of UTRF,CysC, Kim-1 and the combination of the three for the diagnosis of kidney injury in type 2 diabetes mellitus were0.800, 0.714, 0.816 and 0.908, respectively, with sensitivities of 0.732, 0.716, 0.803 and 0.876 and specificities of0.815, 0.821, 0.825 and 0.882, respectively. The sensitivity and specificity of the combined detection of UTRF,CysC and Kim-1 were higher than those of the single-factor detections. Conclusion: The combined detection of UTRF, CysC and Kim-1 can improve the diagnostic efficacy for kidney injury in type 2 diabetes mellitus.
