女性不同生理周期与突发性聋的相关性研究 被引量:4

Correlation Between Menstrual Cycles and Sudden Deafness

摘要 突发性聋,又称突发性感音神经性聋(Sudden sensorineural hearing loss,SSNHL),其病因存在诸多假说。其中一种认为性激素水平的改变可能增加血栓形成风险并阻断耳蜗微循环,导致SSNHL的发生。女性处于不同生理周期阶段时,体内性激素水平会发生显著变化。一般认为,雌激素可以兴奋神经元,对听力有保护作用,而孕激素可以拮抗雌激素的保护作用。本研究探究女性不同生理周期与SSNHL的相关性,结果表明:1.不同时期患者平均听阈值存在显著差异(P=0.0026),且卵泡期患者平均听阈值显著低于月经期(P=0.001)、黄体期(P=0.002)和绝经期(P=0.001);2.患者处于不同时期与SSNHL严重程度之间存在相关性(χ^2=31.145,df=3,P<0.001),且卵泡期SSNHL严重程度最低;3.在不同时期患者SSNHL分型的分析表明卵泡期患者低频下降型较多而高频下降型较少;黄体期高频下降型较多。本研究结果可以在一定程度上阐释性激素对SSNHL的影响,今后将更进一步探究性激素水平与SSNHL的相关性,从而为SSNHL的诊断和治疗,提供更有力的支持。 Sudden deafness,also known as sudden sensorineural hearing loss(SSNHL),has many hypotheses about its etiology.One of them believes that changes in sex hormone levels may increase the risk of thrombosis and blockage of cochlear microcirculation,leading to the occurrence of SSNHL.The levels of sex hormones in a woman’s body change significantly during different phases of the menstrual cycle.It is generally believed that estrogen can excite neurons and has a protective effect on hearing,while progesterone can antagonize the protective effect by estrogen.This study is focused on the correlation between different phases of the menstrual cycle and SSNHL in women.Our results showed:1)significantly different average hearing thresholds during different menstrual cycle phases(P=0.0026),which were significantly lower in the follicular phase than that in the menstrual(P=0.001),luteal(P=0.001)and menopause(P=0.001)phases;2)a correlation between menstrual cycle phases and the severity of SSNHL(χ^2=31.145,df=3,P<0.001),which was the mildest in the follicular phase;and 3)more low-frequency and less high-frequency deafness in the follicular phase and more high-frequency deafness in the luteal phase.The results of this study may explain the effect of sex hormones on SSNHL to a certain extent,and prompt further studies on the relationship between sex hormone levels and SSNHL in order to improve SSNHL diagnosis and treatment.
