绝经后骨质疏松老年女性血清Hepcidin、BMP6、s-HJV水平与骨折的相关性分析 被引量:11

Correlation between serum levels of hepcidin,BMP6,sHJV and fracture in elderly women with postmenopausal osteoporosis

摘要 目的分析老年绝经后骨质疏松(PMO)女性血清铁调素(Hepcidin)、骨形成蛋白6(BMP6)、可溶型铁调素调节蛋白(s-HJV)水平与骨折关系。方法选取150例老年PMO女性作为研究组,及同期进行体检的170例老年无PMO健康女性作为对照组。使用酶联免疫吸附法(ELISA)检测血清Hepcidin、BMP6、s-HJV水平;使用电化学发光全自动免疫分析仪检测血清Ⅰ型前胶原氨基端前肽(PINP)、骨钙素N端中分子(N-MID)、Ⅰ型胶原羧基端肽β特殊序列(β-CTX)及25-羟基维生素D[25-(OH)D]水平,使用速率法检测钙离子(Ca2+)水平;使用双能X线骨密度仪测量腰椎L1~4及左股骨颈BMD值。结果研究组血清Hepcidin、BMP6水平均显著低于对照组(P<0.05),血清s-HJV水平明显高于对照组(P<0.05)。研究组血清PINP、N-MID、β-CTX水平均明显高于对照组(P<0.05),血清25-(OH)D、Ca2+水平均显著低于对照组(P<0.05)。研究组腰椎L1~4及左股骨颈BMD均明显低于对照组(P<0.05)。研究组血清Hepcidin、BMP6水平与PINP、N-MID、β-CTX水平均呈负相关(P<0.05),与血清25-(OH)D、Ca2+水平及腰椎L1~4、左股骨颈BMD均呈正相关(P<0.05);血清s-HJV水平与PINP、N-MID、β-CTX水平均呈正相关(P<0.05),与血清25-(OH)D、Ca2+水平及腰椎L1~4、左股骨颈BMD均呈负相关(P<0.05)。多因素Logistic回归发现,PINP、N-MID、25-(OH)D、Ca2+、腰椎L1~4 BMD、Hepcidin、BMP6、s-HJV均是老年PMO女性发生骨折的独立危险因素。结论血清Hepcidin、BMP6、s-HJV水平均可作为预测老年PMO女性骨折发生的有效指标。 Objective To investigate the correlation between the serum levels of hepcidin,bone morphogenetic protein 6(BMP6),soluble ferritin regulatory protein(sHJV)and fracture in elderly women with postmenopausal osteoporosis(PMO).Methods A total of 150 elderly women with PMO were enrolled as research group,at the same time,the other 170 healthy elderly women without PMO who underwent physical examination were enrolled as control group.The serum levels of hepcidin,BMP6 and sHJV were detected by enzyme linked immunosorbent assay(ELISA),the serum levels of typeⅠprocollagen amino terminal propeptide(PINP),osteocalcin N terminal intermediate molecule(NMID),typeⅠcollagen carboxy terminal peptideβspecific sequence(β-CTX)and 25 hydroxyvitamin D[25(OH)D]were detected by electrochemiluminescence full automatic immunoassay,and the calcium ion(Ca2+)levels were measured by rate method,moreover,the BMD values in lumbar spine L1~4 and left femoral neck were measured by dual energy X ray absorptiometry.Results The serum levels of hepcidin and BMP6 in research group were significantly lower than those in control group(P<0.05),however,the serum levels of sHJV in research group were significantly higher than those in control group(P<0.05).Moreover the serum levels of PINP,NMID andβ-CTX in research group were significantly higher than those in control group(P<0.05),but the serum levels of 25(OH)D and Ca2+in research group were significantly lower than those in control group(P<0.05).The BMD values in L1~4 lumbar spine and left femoral neck in research group were significantly lower than those in control group(P<0.05).In addition the serum levels of hepcidin and BMP6 in research group were negatively correlated with the levels of PINP,NMID andβ-CTX(P<0.05),however,which were positively correlated with the serum levels of 25(OH)D,Ca2+,the BMD values in lumbar L1~4 and left femoral neck(P<0.05).The serum levels of sHJV were positively correlated with the serum levels of PINP,NMID andβ-CTX(P<0.05),but,which were negatively correlated with the serum levels of 25(OH)D,Ca2+,and the BMD values in lumbar L1~4 and left femoral neck(P<0.05).Multivariate logistic regression showed that PINP,NMID,25(OH)D,Ca2+,L1~4 BMD,hepcidin,BMP6 and sHJV were the independent risk factors for fracture in elderly women with PMO.Conclusion The serum levels of hepcidin,BMP6 and sHJV can be regarded as the effective indexes for predicting the occurrence of fracture in elderly women with PMO.
