摘要
目的观察蚕沙外熨治疗糖尿病胃轻瘫(DGP)的临床疗效及安全性。方法采用随机数字表法将110例患者分为观察组和对照组各55例。2组均进行糖尿病西医常规治疗,观察组在常规治疗同时予蚕沙外熨,对照组在常规治疗同时予热水袋外熨,2组均连续治疗2周,随访2周。观察2组胃轻瘫主要症状指数(GCSI)评分、中医症状评分、胃排空率、空腹血糖(FBG)、餐后2 h血糖(2 h PBG)、糖化血红蛋白(HbA1c)和失眠严重程度指数(ISI),比较2组临床疗效,并监测安全性指标。结果观察组、对照组分别脱落3、5例。治疗后及随访时,观察组总有效率分别为71.2%(37/52)、80.8%(42/52),对照组分别为62.0%(31/50)、68.0%(34/50),观察组明显优于对照组(P<0.05)。与本组治疗前比较,2组治疗后及随访时GCSI评分、中医症状评分、胃排空率、FBG、2 h PBG、ISI明显改善(P<0.05),2组随访时HbA1c无明显改善(P>0.05);2组治疗同一时点比较,观察组治疗后及随访时GCSI评分、中医症状评分、胃排空率、FBG、2 h PBG和ISI明显优于对照组(P<0.05)。2组均未发生不良反应。结论蚕沙外熨治疗DGP疗效较好,可明显改善患者GCSI评分、中医症状评分、胃排空率及FBG、2 h PBG水平,并可改善睡眠状况。
Objective To observe the clinical efficacy and safety of the treatment of diabetic gastroparesis(DGP)by hot compressing with silkworm excrement.Methods Totally 110 cases of DGP were divided into observation group and control group,with 55 cases in each group.Both groups received conventional treatment of diabetes by Western medicine.The observation group received hot compressing with silkworm excrement,and the control group received hot compressing with hot water bag.Gastroparesis cardinal symptom index(GCSI)score,TCM symptom score,gastric emptying rate,fasting blood glucose(FBG),2 h postprandial blood glucose(2 h PBG),glycated hemoglobin(HbA1c)and insomnia severity index(ISI)of both groups were observed.Clinical efficacy of both groups was compared.Safety indicators were monitored.Results The observation group and control group lost 3 and 5 cases,respectively.After treatment and during follow-up,the total effective rates of the observation group were 71.2%(37/52)and 80.8%(42/52),and the control group were 62.0%(31/50)and 68.0%(34/50),respectively.The observation group was significantly better than the control group(P<0.05).Compared with before treatment,the GCSI score,TCM symptom scores,gastric emptying rate,FBG,2 h PBG,ISI of the two groups were significantly improved after treatment and during follow-up(P<0.05),while HbA1c was not significantly improved in the two groups during follow-up(P>0.05).Compared at the same time point,the observation group had better GCSI score,TCM symptom score,gastric emptying rate,FBG,2 h PBG and ISI after treatment and follow-up than the control group(P<0.05).No adverse reactions occurred in either group.Conclusion The treatment of DGP by hot compressing with silkworm excrement has a better efficacy,which can significantly improve the GCSI score,TCM symptom scores,gastric emptying rate,FBG,2 h PBG,and sleeping.
