摘要
目的系统评价中药热奄包用于糖尿病胃轻瘫患者的疗效,为中医外治法在糖尿病领域的进一步推广应用提供科学依据。方法检索中国知网、万方数据库、维普网、SinoMed、PubMed、Web of Science、Cochrane、Embase、Ovid、Proquest、EBSCO11个数据库,全面检索中药热奄包治疗糖尿病胃轻瘫的随机对照试验,检索时限设定为建库至2025年1月2日。由2名研究者独立筛选文献,数据分析采用RevMan5.3软件完成。结果共计纳入12篇文献,Meta分析结果显示,中药热奄包观察组在症状评分(腹胀、早饱、恶心呕吐、腹痛)、胃动力学指标(胃排空率、胃收缩频率及胃排空时间)、血糖水平(空腹血糖及餐后2h血糖)、胃肠激素指标(胃动素、瘦素、胃泌素)、临床疗效方面均优于对照组,P<0.05。糖化血红蛋白与生长抑素水平未呈现统计学差异,尚不足以证实中药热奄包对这两项指标具有明确疗效。结论中药热奄包在改善糖尿病胃轻瘫患者的临床症状、血糖水平等方面具有显著优势,建议将其纳入大范围的临床治疗路径中。
Objective To systematically evaluate the therapeutic efficacy of Chinese medicine hot compress in patients with diabetic gastroparesis(DGP),and to provide a scientific basis for further promotion and application of traditional Chinese medicine external therapies in diabetes management.Methods Randomized controlled trials(RCTs)on Chinese medicine hot compress for DGP were comprehensively retrieved from 11 databases,including CNKI,Wanfang Data,VIP,SinoMed,PubMed,Web of Science,Cochrane,Embase,Ovid,Proquest,and EBSCO,from database inception to January 2,2025.Two researchers independently screened the literature,and data analysis was performed using RevMan 5.3 software.Results A total of 12 articles were included.Meta-analysis showed that the Chinese medicine hot compress group was superior to the control group in improving symptom scores(abdominal distension,early satiety,nausea and vomiting,abdominal pain),gastric motility indicators(gastric emptying rate,frequency of gastric contraction,gastric emptying time,gastrin),blood glucose levels(FBG and 2hPBG),gastrointestinal hormone levels(motilin,leptin),and clinical efficacy(all P<0.05).However,no significant differences were observed in glycated hemoglobin(HbA1c)and growth inhibitor levels,indicating insufficient evidence to support the efficacy of Chinese medicine hot compress on these two indicators.Conclusion Chinese medicine hot compress demonstrates significant advantages in improving clinical symptoms and glycemic control in patients with DGP,it should be widely incorporated into clinical treatment pathways.
