摘要
目的研究坤泰胶囊联合艾灸治疗早发性卵巢功能不全的临床效果。方法将在深圳市南山区西丽人民医院于2017年1月-2018年6月确诊的早发性卵巢功能不全80例患者作为研究对象。采取简单随机分组法分为A组、B组、C组、D组,每组各20例。A组给予坤泰胶囊4粒/d,3次/d,悬灸神阙穴、关元穴、气海穴和足三里穴;B组给予单独坤泰胶囊口服方法同A组;C组单独给予艾灸法同A组;D组给予安慰剂维生素E。治疗2个疗程后行指标检测:以放射免疫法测定卵泡刺激激素(FSH)、雌二醇(E2)、黄体生成素(Luteinizing hormone,LH);以双抗体夹心酶免疫吸附法测定抗苗勒氏管激素及人抑制素B(INH-B);阴道超声检查窦卵泡数;月经前三天检测卵巢直径2~9 mm的基础卵泡个数。结果干预前各组患者的FSH、E2、LH、抗苗勒氏管激素及INH-B水差异无统计学意义(P>0.05),治疗2个疗程后各组的FSH、LH、INHB指标水平明显下降,E2及抗苗勒氏管激素水平明显增高(P<0.05),但A组优于余下三组,B组优于C组与D组(P<0.05),C组与D组间无明显差异(P>0.05);各组患者的窦卵泡个数与卵巢搏动指数差异无统计学意义(P>0.05),治疗两个疗程后各组的窦卵泡个数增加,卵巢搏动指数下降(P<0.05),但A组指标优于余下3组,B组优于C组与D组(P>0.05),C组与D组相比差异无统计学意义(P>0.05);结论坤泰胶囊联合艾灸疗法具有更好的治疗卵巢早衰临床效果,能够改善激素水平及卵巢功能。
Objective To study the clinical effect of Kuntai capsule combined with Moxibustion in the treatment of early-onset ovarian insufficiency. Methods A total of 80 cases of early ovarian insufficiency diagnosed from January,2017 to June,2018 were included in the study. Group A was given Kuntai Capsule 4 capsules per day,3 times per day,suspended moxibustion at Shenque,Guanyuan,Qihai and Zusanli points. Group B was given Kuntai Capsule orally as group A, group C was given moxibustion alone as group A,and group D was given placebo vitamin E. After two courses of treatment,the following indicators were detected:(1)follicle stimulating hormone(FSH),estradiol E2 and luteinizing hormone(LH)were measured by radioimmunoassay;(2)anti-Mullerian hormone and human inhibin B(INH-B)were measured by double antibody sandwich enzyme immunosorbent assay;(3)the number of sinus follicles was examined by transvaginal ultrasound;(4)Number of follicles of ovarian diameter 2-9 mm three days before menstruation was measwed. Results There were no significant differences in the levels of FSH,E2,LH,anti-Mullerian hormone and INH-B in each group before intervention,P>0.05. After two courses of treatment,the levels of FSH, LH and INH-B in each group decreased significantly,while the levels of E2 and anti-Mullerian hormone increased significantly(P<0.05),but group A was better than the other three groups,group B was better than group C and group D,P<0.05,and there was no significant difference between group C and group D,P>0.05. There was no significant difference between the number of sinus follicles and ovarian pulsation index(P>0.05). After two courses of treatment,the number of sinus follicles in each group increased,the ovarian pulsation index decreased,P<0.05,but the index in group A was better than that in the remaining three groups,group B was better than that in group C and group D,and there was no significant difference between group C and group D,P>0.05. Conclusion Kuntai capsule combined with moxibustion therapy has better clinical effect on premature ovarian failure. The effect can improve hormone level and ovarian function.
