摘要
目的观察补肾中药益真Ⅰ、Ⅱ号胶囊对高龄不孕妇女围排卵期卵巢间质血流的影响。方法高龄不孕妇女48例随机分成中药治疗组(26例)和西药克罗米芬治疗组(22例),观察围排卵期经阴道多普勒超声下卵子大小、形态及卵巢间质血流的变化。结果两组治疗前后不同时期的子宫内膜厚度均无显著性变化,中药组卵泡增长速率较治疗前加快,卵泡饱满透亮,小卵泡排卵明显减少,卵泡与子宫内膜发育同步。中药治疗组周期第12～16天RI、PI值均显著减低,治疗前后比较差异具有显著性(P<0.05),西药克罗米芬治疗组卵泡发育与子宫内膜不同步,治疗前后RI、PI无显著变化。治疗后中药治疗组RI、PI也较西药克罗米芬治疗组显著下降, 差异具有显著性( P <0.05) 。中药治疗组受孕率 36.3%; 西药克罗米芬治疗组受孕率 23.52%, 两组比较差异有显著性( P <0.05) 。结论围排卵期卵巢间质血流的改变间接反映卵子质量、卵巢储备的提高。补肾中药改善卵子质量, 提高卵巢储备的作用, 可通过经阴道多普勒超声下卵泡的生长发育及间质血流的改变直观反映。
[Objective] To observe the effect of kidney-tonifying traditional Chinese herbs Yizhen Ⅰ, Ⅱ capsules on periovulatory ovarian interstitial blood flow in infertile women of advanced reproductive age. [Methods] Forty-eight infertile women of advanced reproductive age were randomly divided into two group： the Chinese medicine group （26 cases） was treated with Yizhen Ⅰ, Ⅱ capsules （treatment group） and the control group （22 cases） treated with clomiphene. The follicle size, morphology and ovarian interstitial blood flow were observed by transvaginal Doppler ultrasound. [Results] Endometrial thickness in different periods had no significant change before and after the treatment in both groups. In the treatment group, follicle growth rate after treatment was accelerated than before treatment, follicle was full translucent, small follicle ovulation noticeably reduced. Meanwhile in the treatment group, resistive index （RI） and pulsatility index （PI） were significantly reduced during the 12^th -16^th day of menstrual cycle, the difference before and after treatment was significant （P 〈0.05）. However, in the control group, RI and PI had no significant change before and after treatment. After Chinese medicine treatment, RI and PI decreased significantly, compared with the control group （P〈0.05）. The pregnancy rate was 36.3% in the treatment group, 23.52% in the control group. The difference between the two groups was significant （P〈0.05）. [Conclusions] The change of ovarian interstitial blood flow in periovulatory period indirectly reflects the improvement of ovum quality and ovarian reserve. The effect that kidney-tonifying traditional Chinese herbs can improve ovum quality and ovarian reserve, can be indirectly reflected by the changes of follicle growth and ovarian interstitial blood flow under transvaginal Doppler ultrasound.
