摘要
目的探讨酚妥拉明联合尼卡地平治疗妊娠高血压对患者血清亮胺酸胺肽酶(LAP)和脂连蛋白(APN)水平的影响。方法以2017年2月至2018年2月收治的120例妊娠高血压患者为研究对象,根据区组随机方法分为试验组和对照组,各60例。对照组给予酚妥拉明治疗,试验组给予酚妥拉明联合尼卡地平治疗。分析治疗前后2组的血压、胎盘血流动力学参数、妊娠结局,以及血清LAP、APN水平变化情况。结果治疗后2组的舒张压和收缩压均降低(P<0.05),且试验组均低于对照组(P<0.05)。试验组的总有效率(96.67%)高于对照组(86.67%)(χ~2=3.927,P=0.048),顺产率也高于对照组(P<0.05),而新生儿早产率、死亡率、窒息率和宫内窘迫率均低于对照组(P<0.05)。治疗10 d后,2组患者的收缩/舒张比值(SD)、脉动指数(PI)和阻力指数(RI)均降低(P<0.05),且试验组均低于对照组(P<0.05);2组的脐动脉时间平均流速(TAV)及血清LAP、APN水平均上升(P<0.05),且试验组高于对照组(P<0.05)。试验组和对照组的不良反应发生率分别为16.67%和20.00%,差异无统计学意义(χ~2=0.223,P=0.637)。结论酚妥拉明联合尼卡地平治疗妊娠高血压,可明显降低患者的血压,有效提高LAP、APN水平,同时还可改善患者妊娠结局及新生儿结局,安全性较高。
AIM To study the effect of phentolamine combined with nicardipine on serum leucine aminopeptidase(LAP)and adiponectin(APN)levels in patients with gestational hypertension.METHODS A total of 120 patients with gestational hypertension from February 2017 to February 2018 were selected as study objects.According to the random method of area group,it was divided into experimental group and control group,with 60 patients in each group.The control group was treated with phentolamine,while the experimental group was given phentolamine combined with nicardipine.The blood pressure,placental hemodynamics,pregnancy outcome,LAP and APN levels in the 2 groups were analyzed before and after treatment.RESULTS After treatment,the diastolic and systolic blood pressure of the 2 groups decreased(P<0.05),and the experimental group was lower than the control group(P<0.05).The total effective rate of the experimental group(96.67%)was higher than that of the control group(86.67%)(χ~2=3.927,P=0.048).The yield rate of the experimental group was higher than that of the control group(P<0.05),and the preterm birth rate,mortality,asphyxia rate and intrauterine distress rate were lower than those of the control group(P<0.05).After 10 days of treatment,the systolic velocity/diastolic velocity ratio(SD),pulse index(PI)and resistance index(RI)in the 2 groups decreased(P<0.05),and the SD,PI and RI in experimental group were lower than those in the control group(P<0.05);the umbilical arterial time average velocity(TAV)and expressions of LAP and APN in the 2 groups increased(P<0.05),and TAV,expressions of LAP and APN in the experimental group were higher than those in the control group(P<0.05).The adverse drug reaction rates of the experimental group and the control group were 16.67%and 20.00%respectively,and there was no statistical difference(χ~2=0.223,P=0.637).CONCLUSION Phentolamine combined with nicardipine in the treatment of gestational hypertension can effectively improve the level of LAP and APN and can significantly reduce the blood pressure of patients,and also can improve pregnancy outcomes and neonatal conditions,average rate of flow(TAV)and serum LAP and APN levels of time.
