摘要
目的探讨仙方活命饮药线引流在肛周脓肿中的应用价值。方法选取2017年10月~2018年10月我院肛周脓肿患者92例,按随机数字表法分为对照组(n=46)与研究组(n=46)。对照组采用凡士林纱条引流,研究组采用仙方活命饮药线引流,共治疗28 d。统计两组恢复情况(创面腐肉脱落时间、愈合时间、住院时间)、临床疗效、治疗后28 d症状积分(创面渗液、创面水肿、创面肉芽)及治疗后7 d、14 d、21 d与28 d疼痛程度(VAS)、治疗前及治疗后28 d血清因子[表皮生长因子(EGF)、碱性成纤维细胞生长因子(bFGF)、转化生长因子-β1(TGF-β1)]水平。结果(1)恢复情况:研究组创面腐肉脱落时间、愈合时间、住院时间短于对照组(P<0.05);(2)临床疗效:研究组总有效率95.65%(44/46)高于对照组的82.61%(38/46)(P<0.05);(3)症状积分:治疗后28 d研究组创面渗液、创面水肿、创面肉芽分值低于对照组(P<0.05);(4)疼痛程度:治疗后7 d、14 d、21 d及28 d研究组VAS分值低于对照组(P<0.05);(5)血清因子:治疗后28 d两组血清EGF、bFGF、TGF-β1水平较治疗前增高,且研究组高于对照组(P<0.05)。结论采取仙方活命饮药线引流治疗肛周脓肿,可减轻疼痛程度,缩短创面恢复用时,利于缓解患者临床症状,改善血清EGF、bFGF、TGF-β1水平,提高疾病整体治疗效果。
Objective To explore the application value of Xianfang live drink drug line drainage in perianal abscess.Methods A total of 92 patients with perianal abscess in our hospital from October 2017 to October 2018 were enrolled.They were divided into control group(n=46)and study group(n=46)according to random number table method.The control group was treated with vaseline gauze drainage,and the study group was treated with Xianfang live drink drug line drainage for 28 days.The statistics of recovery status of the two groups(the time off carrion,healing time,hospitalization time),clinical efficacy,symptom scores at 28 days after treatment(wound surface exudate,wound edema,wound granulation)and pain degree(VAS)at 7 d,14 d,21 d after treatment,serum factor levels(EGF,BFGF,TGF-β1)before treatment and at 28 days after treatment were performed.Results(1)Recovery:The time off carrion,healing time and hospitalization time of the study group was shorter than that of the control group(P<0.05).(2)Clinical efficacy:The total effective rate of the study group was 95.65%(44/46),higher than 82.61%(38/46)of the control group(P<0.05).(3)Symptom score:The wound exudation,wound edema and wound granulation score of the study group were lower than the control group at 28 days after treatment(P<0.05).(4)Pain degree:VAS scores of the study group were lower than those of the control group at 7 d,14 d,21 d and 28 d after treatment(P<0.05).(5)Serum factors:Serum EGF,bFGF,TGF-β1 levels at 28 d after treatment was higher than that before treatment,and the levels of the study group were higher than those of the control group(P<0.05).Conclusion The Xianfang live drink drug line drainage in the treatment of perianal abscess can reduce the degree of pain and shorten the recovery of wounds,which is beneficial to alleviate the clinical symptoms of patients,improve the levels of serum EGF,bFGF and TGF-β1 and improve the overall therapeutic effect of the disease.
