摘要
目的:观察盐酸西那卡塞联合骨化三醇注射液治疗继发性甲状旁腺功能亢进症(SHPT)的临床疗效。方法:选择2017年4月—2019年3月住院尿毒症患者82例,按照随机数字表法分为对照组(41例)和观察组(41例)。对照组静脉注射骨化三醇注射液,观察组在对照组基础上加用盐酸西那卡塞,于治疗第2周第4周、第8周、第12周、第16周后检测两组患者甲状旁腺激素(PTH)、碱性磷酸酶(ALP)、血钙(SCa)、血磷(SP)、血红蛋白(Hb)水平,超声检查两组甲状旁腺体积并观察两组临床疗效。结果:治疗前两组PTH,ALP,SCa,SP,Hb及甲状旁腺体积比较,差异无统计学意义(P>0.05);治疗后两组PTH,ALP,SP和甲状旁腺体积均降低,且观察组降低幅度更大(P<0.05);治疗后对照组SCa与治疗前比较,差异有统计学意义(P<0.05),观察组SCa与治疗前比较,差异无统计学意义(P>0.05);治疗后两组Hb均升高,且观察组升高幅度更大,差异有统计学意义(P<0.05)。观察组总有效率高于对照组,差异有统计学意义(P<0.05)。结论:盐酸西那卡塞联合骨化三醇注射液能有效降低SHPT患者的PTH,ALP,SP和甲状旁腺体积,升高Hb,提高临床疗效。
Objective:To observe the clinical efficacy of cinacalcet hydrochloride combined with calcitriol injection in the treatment of secondary hyperparathyroidism(SHPT).Methods:From April 2017 to March 2019,82 patients with uremia in our hospital were selected and divided into control group(41 cases)and observation group(41 cases)according to the random number table method.The control group was given intravenous calcitriol injection.The observation group was treated with cinacalcet hydrochloride on the basis of the control group.The parathyroid hormone(PTH)was detected in the two groups after 2,4,8,12,and 16 weeks of treatment.Alkaline phosphatase(ALP),serum calcium(SCa),serum phosphorus(SP),hemoglobin(Hb)levels,ultrasound examination of the two groups of parathyroid gland volume and observed the clinical efficacy of the two groups.Results:There was no significant difference in the volume of PTH,ALP,SCa,SP,Hb and parathyroid gland between the two groups before treatment(P>0.05).After treatment,the volume of PTH,ALP,SP and parathyroid gland decreased.The decrease of the observation group was greater(P<0.05).The SCa of the control group increased after treatment,but the difference of SCa in the observation group was not statistically significant(P>0.05).After treatment,the Hb of the two groups increased,and the observation group increased.The amplitude was larger(P<0.05).The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Cinacalcet hydrochloride combined with calcitriol injection can effectively reduce the volume of PTH,ALP,SP and parathyroid gland in patients with SHPT,increase Hb and improve clinical efficacy.
