慢性肾脏病甲状旁腺激素代谢异常的研究进展 被引量:9

摘要 继发性甲状旁腺功能亢进(SHPT)是慢性肾脏病(CKD)常见的并发症之一。随着肾小球滤过率的下降,循环中甲状旁腺激素(PTH)水平逐渐升高。CKD患者的PTH分泌和代谢紊乱,以及靶器官对PTH反应性降低在SHPT的发生发展中发挥重要作用,近年来其相关机制也得到进一步阐明。由于CKD患者PTH代谢和功能异常,PTH作为CKD预后判断标志物的价值仍存在争议。本文将对CKD患者存在的PTH代谢异常及其可能机制进行综述。 Secondary hyperparathyroidism (SHPT) is one of the common complications of chronic kidney disease (CKD). Circulating parathyroid hormone (PTH) was elevated secondary to the decline of glomerular filtration rate. Abnormal secretion and metabolism of PTH, along with end-organ hyporesponsiveness to PTH in CKD played a critical role in the development of SHPT. The relative mechanism had been further clarified recently. However, there are controversies about PTH as a prognostic biomarker in CKD due to its metabolic and functional abnormity. This article will summarize recent studies about abnormal metabolism and function of PTH and the potential mechanism in CKD patients.
