摘要
目的:观察地佐辛联合丙泊酚用于老年患者无痛肠镜检查的有效性及安全性。方法:将2017年9月~2018年4月在许昌市中心医院内镜中心行无痛肠镜检查的老年患者180例,依据麻醉方案不同分为观察组和对照组,每组90例。观察组应用地佐辛联合丙泊酚麻醉;对照组单用丙泊酚麻醉。观察两组平均动脉血压(Mean artery pressure,MAP)、血氧饱和度(Pulse oxygen saturation,Sp O2)、呼吸频率(Respiratory rate,RR)、心率(Heart rate,HR)等血流动力学指标,记录两组丙泊酚用量、意识恢复时间、定向恢复时间、满意程度及药品不良反应发生情况。结果:两组均顺利完成无痛肠镜检查,检查期间两组MAP、Sp O2、RR、HR均有所下降,检查期间两组MAP、HR差异有统计学意义(P<0.05);观察组满意度明显高于对照组,差异有统计学意义(P<0.05);观察组丙泊酚用量、意识恢复时间小于对照组,差异有统计学意义(P<0.05);两组发生的药品不良反应均轻微,观察组药品不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论:地佐辛联合丙泊酚用于老年患者无痛肠镜检查安全有效,同时可提升满意程度。
Objective:To observe the efficacy and safety of dezocine combined with propofol for painless colonoscopy in elderly patients.Methods:From September 2017 to April 2018,180 elderly patients undergoing painless colonoscopy in endoscope center of xuchang central hospital were divided into the observation group and the control group based on the anesthetic scheme,with 90 cases in each group.The patients in the observation group were treated with dezocine combined with propofol and the control group was treated with propofol alone.The mean arterial blood pressure(MAP),pulse oxygen saturation(SpO2),respiratory rate(RR),heart rate(Heart),and other hemodynamic indexes were observed in the two groups.The propofol dosage、the consciousness and directed recovery time from anesthesia,degree of satisfaction,incidence of adverse drug reactions in two groups were recorded.Results:All the patients in two groups were successfully completed colonoscopy inspection,and hemodynamic parameters such as MAP,SpO2,RR and HR were decline during the inspection.The difference with MAP and HR during the examination in two groups was statistically significant(P<0.05).The satisfaction degree of the patients in the observation group was higher than the control group,and the difference was statistically significant(P<0.05).The propofol dosage and the consciousness recovery time from anesthesia in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The adverse drug reactions in the two groups were mild,and the incidence of adverse drug reaction in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:Dezocine combined with propofol is safe and effective for painless colonoscopy in elderly patients,and can improve patients'satisfaction.
