摘要
目的 :探讨糖尿病黄斑水肿的光学相干断层扫描 (opticalcoherencetomography ,OCT)与多焦视网膜电图 (multifocalelec troretinography ,MERG)的影响。方法 :糖尿病黄斑水肿患者 3 9人 60只眼 ,经眼底检查黄斑部视网膜增厚大于 1PD、有硬性渗出 ,荧光造影检查黄斑部有荧光渗漏、OCT检查中心凹厚度大于 170um。除外影响黄斑的其他眼底疾病 ,进行OCT检查中心凹、经中心凹 2 2 5、 3 15经线的两侧约 10 0 0um处视网膜神经上皮层处测量。常规进行MERG检查。 18人 3 5只与年龄相配的正常眼对照组以同样方法行OCT及MERG贴检查与糖尿病黄斑水肿眼进行对比分析。结果 :糖尿病黄斑水肿组中心凹、鼻上、鼻下、颞上、颞下视网膜神经上皮层厚度分别为 3 2 0 62± 188 82um、 3 5 4 78±12 0 92um、 3 46 12± 12 0 43um、 3 5 3 17± 10 0 68um、 3 5 0 0± 114 81um ,对照组分别为 13 9 5 5± 5 0 1um、 2 84 13± 15 0 4um、2 85 48± 16 2 3um、 2 81 84± 15 89um、 2 86 94± 14 13um ,单因素方差分析中心凹及各象限组间均有极显著性差异 (P值均 <0 0 1) ,中心 1环和 4环的N1、P1波潜伏期和振幅密度糖尿病水肿组与对照组之间有极显著性差异 ,P值均 <0 0 0 1。黄斑部的神经上皮层厚度与MERG的N 1?
Objective:To determine the correlation between tomographic feature and visual function of the eye with diabetic macular edema. Methods:Optical coherence tomography (OCT) and multifocal electroretinogram (MERG) were run on 60 eyes of 39 cases of diabetic macular edema and 35 eyes of 18 age-matched normal volunteers.The patients with other retinal diseases were excluded. Results:The neuroepithelium of fovea,and some parts of the retina with diabetic macular edema were significantly thicker than that of the controls (P<0 01).The MERG showed abnormal N1 and P1 waves in ring 1 and ring 4,compared with normal values (P<0 001).A positive correlation was found between fovea thickness and latency of P1 wave in ring 1(r=0 36,P=0 005).The regression equation was Y=-127 98±15 10X.The positive correlations were significantly found between neurosensory thickness and the latencies of N1 and P1 waves in ring 4. Conclusions:The thickness of retinal neuroepithelium in eyes with diabetic macular edema would increase significantly compared with normal eyes.The increased thickness could affect the latencies of N1 and P1 waves of MERG.There is a linear correlation between the status of macular edema and the latency of P1 wave.
