摘要
"三医联动"下的家庭医生制度改革是推进我国新一轮综合医改向纵深发展的重要方向和基础支撑。作为综合医改试点省市之一,上海家庭医生制度"1+1+1"签约服务在加快推进分级诊疗制度建设方面取得了阶段性进展。本文通过梳理"三医联动"背景下上海市家庭医生制度的改革实践,从资源协同与技术支撑、社区首诊与有序管理、政策矛盾与目标导向、家庭医生考核与激励、信息共享与健康共建五个方面总结了制度运行面临的政策瓶颈,并提出针对性建议,以期为深化我国家庭医生制度的改革与发展提供经验借鉴。
The reform of family doctor system under the joint of medical service, medication and medical insurance is an important direction and basic support to promote the new round of comprehensive medical reform in China. As one of the pilot provinces of comprehensive medical reform, Shanghai Family Doctor System ＂1 ＋ 1 ＋ 1＂ contracting service has made phased progress in accelerating the construction of tiered medical system. This paper summarizes the policy bottlenecks facing the operation of Shanghai Family Doctor System from ff ve aspects： resource coordination and technical support, community ff rst diagnosis and orderly management, policy contradiction and goal orientation, family doctor assessment and incentive, information sharing and health co-construction, and put forward targeted policy recommendations, in order to provide experience for deepening the reform and development of family doctor system in China.
