摘要
目的调查家庭医生签约服务背景下社区老年慢性病患者焦虑、抑郁情况并分析其影响因素,为开展社区老年慢性病患者的心理支持提供参考。方法抽取徐州市2个社区家庭签约服务的276例社区老年慢性病患者为研究对象,采用问卷调查所有患者的一般情况、焦虑情况及抑郁情况,分析焦虑、抑郁的影响因素。结果276例老年慢性病患者焦虑检出率为13.77%,抑郁检出率22.83%,其焦虑检出率受性别、家庭关系、慢性病种数、慢性病病程、是否患关节炎、是否患冠心病的影响,差异有统计学意义(P<0.05),抑郁检出率受家庭关系、慢性病病程、患慢性支气管炎、患冠心病影响,差异有统计学意义(P<0.05)。通过二元logistic回归分析得出,老年慢性病患者的焦虑检出率与性别、慢性病病种数其显著相关(P=0.01),抑郁检出率与家庭关系显著相关(P=0.04)。结论建议社区医院加强疾病预防知识的宣教,关注女性以及家庭关系差的老年慢性病患者。
Objective To investigate the anxiety and depression status of the elderly with chronic diseases in community under the background of contracted family doctor services,and to analyze the influencing factors for providing reference for psychological support of the elderly with chronic diseases in community.Methods Totally 276 elderly people with chronic diseases in two community health service centers in Xuzhou using Questionnaire were recruited,and their general situation,anxiety status and depression status were investigated by questionnaire,simultaneously the influence factor of anxiety and depression were analyzed.Results The prevalence rate of anxiety and depression was 13.8% and22.8%,respectively,in the 276 cases of elderly patients with chronic diseases.The prevalence rate of anxiety was related with gender,family relationships,the number of chronic diseases,the course of chronic diseases,arthritis,coronary disease(all P<0.05).The prevalence rate of depression was related with family relationships,the course of chronic diseases,chronic bronchitis,coronary disease(all P<0.05).Binary logistic regression showed that the prevalence rate of anxiety was related with gender and number of chronic diseases(P=0.01), the prevalence rate of depression was related with family relationship(P=0.04).Conclusion It is suggested that community health service centers should strengthen disease prevention education and pay more attention to women and elderly people with chronic diseases who have poor family relationship.
