摘要
目的:建立Sysmex XE-5000血液分析仪检测健康成人静脉血血小板五参数(PLT、PCT、MPV、PDW、P-LCR)的参考区间,探讨适合该实验室的生物参考区间.方法:参照美国临床和实验室标准协会(NCCLS)C28-A2文件推荐方法,用Sysmex XE-5000血液分析仪对1452例健康成人静脉血的血小板五参数进行检测,SPSS19.0对结果进行统计学处理.五参数均为非正态性分布,采用百分位数P2.5~P97.5作为生物参考区间.男女组间对比采用非参数秩和检验.结果:PLT、PCT在不同性别间差异有统计学意义(P〈0.05),应作性别分组处理.MPV、PDW、P-LCR在不同性别间,差异无统计学意(P〉0.05),可以合并参考区间.PLT与PCT有显著性相关(r=0.923),MPV、PDW、P-LCR三者间相关性显著.新建立的生物参考区间为:PLT男(×109/L)123.15~302.70、 女(×109/L)143.20~349.00;PCT(%)男13.00~30.00、 女15.00~34.00;MPV(fL)8.90~12.47;PDW(%)9.40~16.56;P-LCR(%)15.72~45.31.随机抽取194例健康体检职工对新建参考区间进行验证,PLT(男性)2.89%落在区间外,PCT(男性)1.92%落在区间外.MPV(女性)、PDW(女性)、P-LCR(女性)各2.22%落在区间外.结论:PLT、PCT参考区间与性别有关,MPV、PDW、P-LCR参考区间与性别无关.新建立的参考区间适用于该实验室,为临床诊疗工作提供了更有意义的参考数据.
Objective: To establish the reference reference interval of the five parameters (PLT, PCT, MPV, PDW and P-LCR) of healthy adults blood platelets by Sysmex XE-5000 blood analyzer, and to explore the biological reference interval that suitable for the laboratory. Methods: According to the recommendation method of the C28-A2 document of the American Association for clinical and laboratory standards (NCCLS) , the five parameters of blood platelets in 1452 healthy adults were detected by Sysmex XE-5000 blood analyzer, and the results were statistically treated by SPSS19.0. The five parameters are non normal distribution, and percentile P25-P975 is used as biological reference interval. Non - parameter rank sum test was used in the comparison between male and famale groups. Results: The difference between PLT and PCT was statistically significant (P〈0.05), and should be treated by gender. There was no significant difference in MPV, PDW and P-LCR between genders (P〉0.05) , and the reference interval could be combined. There was a significant correlation between PLT and PCT (r=0.923). MPV, PDW and P-LCR were significantly correlated. The newly established biological reference ranges are: PLT male (×109/L) 123.15-302.70, female (×109/L) 14320-349.00; PCT (0/0) male 13.00-30.00, female 15.00-34.00; MPV (fL) 8.90- 12.47; PDW (%) 9.40-16.56; P-LCR (0/6) 15.72-45.31. 194 healthy persons were randomly selected to verify the new reference interval. PLT (male, 2.89%) and PCT (male, 1.92%) drooped outside the interval. The rate in either MPV (female) , PDW (female) or P-LCR (female) was 2.22% Conclusion: The reference intervals of PLT and PCT were related to gender. The reference intervals ofMPV, PDW and P-LCR were independent of gender. The newly established reference interval is suitable for the laboratory, which could provide more meaningful reference data for clinical diagnosis and treatment.
