摘要
目的:探讨结直肠癌患者根治术前血清miR-193a-3p和糖类抗原19-9(CA19-9)表达水平与临床病理特征及预后的关系。方法:选取2010年1月至2012年6月在武汉大学人民医院行根治性切除术的216例结直肠癌患者作为研究组。另选取50例正常人作为对照组。采用实时荧光定量PCR(qPCR)法检测患者术前血清中miR-193a-3p的相对表达量,酶联免疫吸附试验(ELISA)方法检测CA19-9表达水平。分析miR-193a-3p、CA19-9表达水平与结直肠癌病理特征及预后的关系。结果:研究组患者miR-193a-3p相对表达量明显低于对照组,CA19-9表达水平明显高于对照组(均P<0.05)。术前miR-193a-3p呈低表达、CA19-9阳性的结直肠癌患者肿瘤分化程度较低、TNM分期较晚,且更易发生淋巴结转移和脉管浸润(P<0.05)。miR-193a-3p低表达组患者复发转移率明显高于miR-193a-3p高表达组,5年生存率明显低于miR-193a-3p高表达组;CA19-9阳性组患者复发转移率明显高于CA19-9阴性组,5年生存率明显低于CA19-9阴性组,差异均有统计学意义(均P<0.05)。结论:结直肠癌患者根治术前血清miR-193a-3p呈低表达,CA19-9呈高表达;miR-193a-3p和CA19-9表达与肿瘤分化程度、临床分期、淋巴结转移等病理特征密切相关,且对患者生存预后有一定的预测作用。
Objective: To explore the the correlation of serum miR-193a-3p and CA19-9 levels and clinicopath- ological characteristics and prognostic of patients with colorectal cancer. Methods: 216 patients with colorectal cancer underwent radical resection in our hospital from January 2010 to June 2012 were selected as a study group. 50 healthy subjects were enrolled as a control group. The expression levels of miR-193a-3p and CA19-9 in serum were respectively detected by re- al-time fluorescence quantitative PCR(qPCR)and enzyme linked immunosorbent assay(ELISA), and the rela- tionship with clinicopathological features and prognosis of patients was analyzed. Results: Compared with the control group, the miR-193a-3p expression level was decreased, and the CA 19-9 level was increased in the study group (P〈0. 05). The patients with lower expression level of miR-193a-3p and positive expression of CA19-9 before surgery were more likely to have lymph node metastasis and vascular infiltration, and lower differentia- tion of tumor, and higher TNM stage (P〈0. 05). The recurrence rate in patients with low-expression of miR-193a-3p and positive expression of CA19-9 was higher than those with high-expression of miR-193a-3p and negative expression of CA19-9 (P〈0. 05). 5-year survival rate in patients with low-expression of miR- 193a-3p and positive expression of CA 19-9 was lower than those with high-expression of miR- 193a-3p and negative expression of CA19-9 (P〈 0. 05). Conclusion: Patients with colorectal cancer had lower expression level of miR-193a-3p and higher serum level of CA19-9. The expressions of miR-193a-3p and CA19-9 were closely related to differentiation of tumor, TNM stage and lymph node metastasis, and were valuable for predicting the prognosis of patients.
