摘要
分析了国内外居家护理发展现状,介绍了居家护理常见的3种开展模式,在医院工作实践基础上,系统阐述了以医联体为依托、以项目建设为平台、以业务联系为纽带的医院-社区-家庭网格化居家护理服务模式,并探讨了医院开展居家护理服务的内容和形式。
The status quo of home care at home and abroad and three common modes of home care are discussed. On the basis of healthcare practice, the paper elaborated the networked home care mode including the hospital - community - family based on hierarchical diagnosis and treatment, project and business, and discussed the content and form of home care provided by hospital.
