摘要
目的研究负压封闭引流(VSD)治疗对下肢骨折创面感染患者下肢微循环及愈合的影响,为下肢骨折创面感染的治疗提供参考依据。方法选取2014年1月-2015年10月接受治疗的46例下肢骨折创面感染患者为研究对象,将患者随机分为对照组(常规骨折创面感染治疗)和观察组(VSD治疗),两组各23例患者,将两组患者治疗前和治疗后1、3、5d的甲襞微循环指标及创面愈合总有效率进行比较;采用SAS6.0软件进行统计分析。结果治疗前两组患者的甲襞微循环指标均无明显差异,观察组治疗后1、3、5d的流态积分、管襻积分、襻周积分及总积分均低于对照组,其他甲襞微循环指标也均好于对照组,创面愈合总有效率高于对照组,差异均有统计学意义(P<0.05)。结论 VSD治疗对下肢骨折创面感染患者下肢微循环及愈合的影响更为积极,更适用于下肢骨折创面感染患者。
OBJECTIVE To explore the influence of VSD on microcirculation and healing of lower limbs of lower limbs fracture patients with wound infection so as to provide guidance for treatment of the lower limbs fracture patients with wound infection.METHODS A total of 46 lower limbs fracture patients with wound infection who were treated from Jan 2014 to Oct 2015 were recruited as the study objects and randomly divided into the control group（conventional treatment of facture wound infection）and the observation group（VSD treatment）,with 23 cases in each group.The nail microcirculation indexes and total effective rate of wound healing were observed and compared between the two groups of patients before the treatment and after the treatment for 1,3,and 5days;the statistical analysis was performed with the use of SAS6.0software.RESULTS There was no significant difference in the nail microcirculation indexes between the two groups of patients before the treatment.The fluid flow integral,loop integral,loop around integral,and total integral of the observation group were lower than those of the control group;other nail microcirculation indexes of the observation group were better than those of the control group.The total effective rate of wound healing of the observation group was higher than that of the control group（P〈0.05）.CONCLUSIONThe VSD has more positive effect on the microcirculation and healing of lower limbs of the lower limbs fracture patients with wound infection and is more suitable for the lower limbs fracture patients with wound infection.
