摘要
目的:观察益气化瘀中药对早期糖尿病肾病(diabetic nephropathy,DN)患者细胞因子水平的影响。方法:98例早期DN患者随机平均分为对照组和治疗组。对照组在常规治疗基础上给予替米沙坦口服,治疗组在对照组基础上给予益气化瘀中药口服。选择40例健康体检者为健康组,比较治疗前DN患者与健康组血清白细胞介素-6(interleukin-6,IL-6)、IL-18、肿瘤坏死因子-α(tumour necrosis factor-α,TNF-α)及血管内皮生长因子(vascular endothelial growth factor,VEGF)水平,比较两组患者治疗前后IL-6、IL-18、TNF-α、VEGF、尿蛋白排泄率(urinary albumin excretion,UAERR)、糖化血红蛋白(glycosylated hemoglobin,Hb A1C)、空腹血糖(fasting blood-glucose,FPG)、血肌酐(serum creatinine,SCr)变化情况。结果:两组患者治疗前Hb A1C、FPG、SCr、UAERR差异无统计学意义(P>0.05),治疗后Hb A1C、UAERR较治疗前明显下降(P<0.05),治疗组低于对照组(t=8.737、14.244,P=0.000、0.000);两组患者治疗前血清IL-6、IL-18、TNF-α及VEGF水平均高于健康组(P<0.05),治疗后两组患者血清IL-6、IL-18、TNF-α及VEGF水平均较治疗前下降(P<0.05),治疗组低于对照组(t=8.737、6.899、3.844、3.216,P=0.000、0.000、0.000、0.001),差异均有统计学意义。结论:益气化瘀中药口服能够降低DN患者血清IL-6、IL-18、TNF-α及VEGF水平,在早期DN的治疗中发挥积极作用。
Objective：To investigate the effect of Chinese medicine Yiqi Huayu on the cell factor levels in patients with early diabetic ne-phropathy（DN）. Methods：98 patients with DN were randomly divided into two groups. The control group（49 cases）was treated with conventional therapy and Telmisartan and the treatment group（49 cases）was treated with Chinese medicine Yiqi Huayu based on the control group. 40 healthy volunteers were selected as the health group,compared the serum levels of interleukin-6（IL-6）,IL-18,tumor necrosis factor alpha（TNF-α）and vascular endothelial growth factor（VEGF）level of the patients with DN in the treatment group and the healthy group. Compared the changes of IL-6,IL-18,TNF-α,VEGF,the rate of urinary protein excretion（UAER）,glycosylated he-moglobin（HbA1C）,fasting blood glucose（FPG）and the serum creatinine（SCr）before and after the treatment. Results：Compared HbA1C,FPG,SCr,UAER of patients before treatment,there was no significant difference（P ﹥ 0. 05）. HbA1C and UAER of treatment were significantly decreased than before treatment（P ﹤ 0. 05）,the treatment group was lower than that of the control group（t = 8. 737, 14. 244,P = 0. 000,0. 000）. Serum IL-6,IL-18,TNF-α and VEGF levels of the two groups were higher than those of the healthy group （P ﹤ 0. 05）. Serum IL-6,IL-18,TNF- α and VEGF levels of the two groups were decreased than before treatment（P ﹤ 0. 05）. The treatment group was lower than that of the control group（t = 8. 737,6. 899,3. 844,3. 216,P = 0. 000,0. 000,0. 000,0. 001）,the differ-ence was statistically significant. Conclusion：Chinese medicine Yiqi Huayu can reduce the serum IL-6,IL-18,TNF- α and VEGF levels of patients with DN and plays a positive role in the treatment of early DN.
