摘要
目的观察肠内营养联合健脾通里中药煎剂对胃癌术后患者早期免疫功能及胃肠功能的影响。方法将60例胃癌手术患者随机分成治疗组和对照组,每组30例。对照组先滴注9.0 g/L氯化钠注射液100 ml,再滴注肠内营养液瑞素500 ml,治疗组先滴注健脾通里中药煎剂100 ml后再滴注瑞素500 ml,每天1次,连续滴注7 d。所有患者术前和空肠营养结束后第1日分别检测血浆免疫球蛋白(IgA、IgM、I gG)、血浆胃泌素(GAS)含量,并观察胃肠功能(肛门排气复常时间、排便复常时间、胃管总引流量和胃管放置时间)的恢复情况。结果与术前比较,术后治疗组血浆免疫球蛋白IgA、IgG和IgM显著升高(均P<0.05);术后对照组血浆免疫球蛋白IgA显著升高(P<0.01),GAS水平显著下降(P<0.01)。治疗组术后IgA、IgG、IgM和GAS水平均高于对照组(P<0.05)。与对照组比较,术后治疗组肛门排气复常时间、排便复常时间、胃管总引流量和胃管放置时间显著缩短(P<0.01)。结论胃癌术后早期肠内营养联合健脾通里中药煎剂,可以改善患者的免疫功能,促进胃肠激素的分泌,促进胃肠功能的早日恢复。
Objective To study the effects of enteral nutrition combined with Jianpi Tongli decoction on the immune function and gastrointestinal function in patients with gastric cancer after operation. Methods 60 patients with postoperative gastric cancer were randomly divided into treatment group（ n = 30） and control group（ n = 30）.While patients in the control group were treated with Sodium Chloride Injection 100 ml and Fresubin 500 ml,patients in the treatment group were treated with Chinese medicine decoction of Jianpi Tongli 100 ml and Fresubin 500 ml. Both of the two groups were treated once daily for 7 days. The levels of the immune function indices（ IgA,IgG,IgM） and the gastrin（ GAS） in the two groups one day before operation and on the eighth day after operation were detected. And the levels of the gastrointestinal function such as the time of passage of gas by anus,the first time of defecation,discharge volume from gastric tube and the placing time of gastric tube were detected. Results The levels of IgA,IgG and IgM in treatment group significantly increased after the operation than those before the operation（ P〈 0. 05）. The levels of postoperative IgA significantly increased（ P〈 0. 01） and the levels of postoperative GAS significantly decreased in control group（ P〈 0. 01）. The levels of postoperative IgA,IgG and IgM in treatment group were significantly higher compared with the control group（ P〈 0. 05）. The time of passage of gas by anus,the first time of defecation,discharge volume from gastric tube and the placing time of gastric tube in treatment groupwere significantly shorter than those in control group（ P〈 0. 01）. Conclusions The early use of enteral nutrition combined with Jianpi Tongli decoction in patients with gastric cancer after operation could improve the immune function,promote the secretion of gastrointestinal hormone and the recovery of gastrointestinal function.
