高分子量细胞角蛋白及前列腺特异性抗原在前列腺腺癌中的表达和意义 被引量:4

摘要 目的 提高对各种前列腺良恶性病变的诊断和鉴别诊断水平。方法 采用免疫组织化学染色方法分别对213例前列腺良恶性病变组织进行高分子量细胞角蛋白34βE_(12)及前列腺特异性抗原(PSA)标记,特异地显示前列腺基底细胞及间接了解基细胞层的完整性。结果 前列腺腺癌基底细胞均丢失,而前列腺增生性病变基底细胞绝大多数保存完好,只有少数Ⅲ级前列腺上皮内肿瘤和非典型性瘤样增生基底细胞不完整。大多数前列腺腺癌PSA低表达,少数分化好的癌组织呈阳性,而良性前列腺增生组织标本中PSA高表达。结论 联合应用特异显示基底细胞的高分子量细胞角蛋白34βE_(12)及间接了解基细胞层完整性的PSA标记物在前列腺良恶性病变的鉴别诊断及肿瘤恶性程度的判断中具有很好的应用价值。 Objective To improve diagnosis and differentiation of benign and malignant prostate lesions. Methods Expression of high molecular weight cytokeratin 34βE12 and prostate specific antigen (PSA) were detected by im-munohistochemical method in 213 cases with benign and malignant prostate tissues, and the basal cells were staind and understand the integrity of the basal cell layer. Results It was demonstrated that the basal cell layer was lost in all prostate adenocarcinomas, but existed in most of the prolifcrative lesions except for atypical adenomatous hyperplasia and grade 3 intracpithelial ncoplasia in which the basal layer was disrupted in some cases. Expression of prostate specific antigen in mast of prostate adenocarcinomas was worse, but expression in benign prostate hyperplasia (BPH) was better. Conclusions The study showed that the 34βE12 anribody and PSA marker were useful in the differential diagnosis of benign and malignant prostate lesions.
