PCNA及nm23的表达与食管鳞癌放射敏感性及预后的相关性研究 被引量:5

Impact of PCNA and nm-23 gene expression on radiosensitivity and prognosis in esophageal squqmous cell carcinoma

摘要 目的 观察PCNA及nm2 3的表达水平与食管鳞癌患者的临床特征、放射敏感性、预后等因素的关系 ,以及在食管鳞癌中PCNA和nm2 3 H1 之间的关系 ,以期找到一个能预测食管鳞癌放射敏感性及预后的生物学指标 ,指导临床治疗。方法 对 1994年至 1997年收治的病理诊断明确、完成全程根治性放射治疗、随访 3年以上的 5 9例中晚期胸段食管鳞癌患者的放射治疗前活检标本 ,用免疫组织化学方法检测其组织切片中增殖细胞核抗原 (PCNA)及肿瘤转移相关基因 (nm2 3)的表达水平。结果 食管鳞癌组织中PCNA及nm2 3的表达水平较正常组织明显增高 (P <0 .0 5 )。PCNA及nm2 3的表达与食管鳞癌患者的性别、年龄、病变部位、病变长度、分化程度等无关 (P >0 .0 5 )。PCNA的表达与食管鳞癌的近期疗效、局部控制率及生存率呈正相关 (P <0 .0 5 )。nm2 3的表达与食管鳞癌的转移呈正相关 (P <0 .0 5 )。相关性分析未显示PCNA与nm2 3表达两者之间有相关性。结论 单纯放射治疗的食管鳞癌中PCNA表达与近期疗效、局部控制率及生存率呈正相关 ;nm2 3表达与食管鳞癌的转移呈正相关。食管鳞癌组织中PCNA与nm2 Objective To define the correlation between the proliferating cell nuclear antigen (PCNA) and metastasis-related gene nm-23-H 1, and to correlate their espressions with clinical features,radiosensitivity and prognostic variables in exophageal squamous cell carcinoma (ESCC), to establish some biological parameters obtained prior to therapy though which we can predict radiosensitivity and outcome.Methods PCNA and nm-23-H 1 expression protein were determined by immunohistochemical technique with formalin-fixed paraffin-embedded specimens from 59 patients with ESCC who had received definitive radical radiation and had been followed up for more than 3 years. The values were assessed by distributions of patients , disease factors, including age,sex,lesion site,legth ,histological grade and prognosis.Results The findings showed that the mean labelling indices of PCNA and nm-23-H 1 were significantly higher in ESCC than in the normal esophageal tissue (P<0.05). The PCNA index was positively associated with local control and survival (P<0.05),while the nm-23-H 1 index with distant metastasis (P<0.05). However,there was no relevant correlation between PCNA and nm-23-H 1 expressions, and no influence of age,sex,tumor site ,lenth or grade was found (P>0.05).Conclusions PCNA and nm-23-H 1 indices can be taken as biological endpoints to predicting therapeutic response, local and systemic control of disease.
