摘要
目的 观察超声乳化白内障吸出术对糖尿病患者黄斑结构的影响。 方法 对比分析 30例行超声乳化白内障吸出术的糖尿病患者手术眼与对侧未手术眼以及 30例无糖尿病行超声乳化白内障吸出术者手术前及术后 1d、1个月时黄斑中心凹厚度的光学相干断层扫描 (optical coherence tomography,OCT)测量资料。 结果 糖尿病患者白内障超声乳化手术眼手术前黄斑中心凹的平均厚度为 (148.5±2 7.7)μm,术后平均厚度为 (2 19.4± 6 8.2 3)μm,二者比较差异有显著性意义 (P<0 .0 5 ) ;未手术眼初次检查黄斑中心凹平均厚度为 (147.4± 2 7.5 )μm,1个月后复查为 (148.2± 2 7.3)μm,二者比较差异无显著性意义 (P>0 .0 5 )。无糖尿病行白内障超声乳化手术眼术前黄斑中心凹平均厚度为 (142 .37± 12 .7) μm,术后为(15 1.9± 2 3.7) μm,二者比较差异无显著性意义 (P>0 .0 5 )。糖尿病患者白内障超声乳化手术组术后新增黄斑水肿 11只眼 ,原有黄斑水肿的 6只眼中 3只眼水肿较术前加重。 结论 糖尿病患者白内障超声乳化吸出术后视网膜厚度明显增加 ,黄斑水肿的发生率较高 ,黄斑水肿的程度较重。
Objective To investigate the effects of phacoemulsification on macula in diabetics. Methods Thirty eyes of cataract in diabetics were chosen randomly for measurement of the thickness of fovea of retina using OCT before phacoemulsification and 1 month after surgery. The other eyes in these patients and 30 eyes of cataract in nondiabetic patients with phacoemulsification were as control. Results In 30 eyes of diabetics with phacoemulsification, the mean fovea thickness were (148.5±27.7) μm preoperatively and (219.4±68.23) μm postoperatively, and the difference was significant (P<0.05). In 30 eyes of diabetics without surgery, the mean foveal thickness were (147.4±27.5) μm preoperatively and (148.2±27.3) μm postoperatively and the difference was not significant (P>0.05). In 30 eyes of cataract in nondiabetic patients, the mean fovea thickness were (142.37±12.7) μm preoperatively and (151.9± 23.7) μm postoperatively and the difference was not significant (P>0.05). In 30 eyes of diabetics with phacoemulsification, 11 eyes had new macula edema after surgery and 3 eyes had significant retinal thickening. In 6 eyes with macular edema before surgery, the macular edema were aggravated in 3 eyes after surgery. The macular structural changes were not found in two control groups. Conclusion The thickness of retina is inreased after phacoemulsification in deabetics,and morbidity and its severity of postopevative macular edema are increased as well.
