二维超声斑点追踪成像对高血压患者左室纵向收缩和舒张功能的评价 被引量:4

摘要 目的应用二维超声斑点追踪成像(2D-STI)技术评价高血压患者左室纵向收缩、舒张功能。方法将原发性高血压病患者60例,依据心肌质量指数分为左室肥厚组(30例)与非左室肥厚组(30例),选取健康者对照组40例。经胸采集左室长轴3个切面图像存盘后,运用QLBA 8.1超声工作站进行脱机分析,检测左室长轴18个心肌节段的纵向收缩峰值应变、应变率(SLs、SrLs),纵向舒张早期峰值应变率(SrLe)。结果高血压病组各心肌节段SLs、SrLs、SrLe均显著低于对照组,差异有统计学意义(P<0.05);左室肥厚组各心肌节段SLs、SrLs、SrLe均显著低于非左室肥厚组,差异有统计学意义(P<0.05)。结论 2D-STI能准确评价高血压病患者舒缩期左室壁纵向舒缩功能,可作为临床评价高血压病患者左室长轴舒、缩功能的新方法。 Objective To assess left ventricular long - axis function in patients with essential hypertension using speckle track- ing imaging. Methods According to the left ventricle mass index (LVMI) ,60 patients with essential hypertension were enrolled to di- vide into the left ventricular hypertrophy group ( n = 30) and the non - left ventricular hypertrophy group ( n = 30 ). The systolic peak longitudinal strain (SLs), systolic peak longitudinal strain rate (SrLs), early diastolic peak longitudinal strain rate (SrLe) of 18 segments of left ventricular four - chamber view, apical two - chamber view and apical long - axis view were obtained by 2D - speckle tracking soft in QLBA 8.1 workstation. 40 health individuals were chosen to be the control group. Results Compared with the control group, SLs, SrLs and SrLe of left ventricular segments in essential hypertension group decreased the differences were statistically significant ( P 〈 0.05 ) ; Compared with the non - left ventricular hypertrophy group, SLs, SrLs and SrLe of left ventricular segments decreased in hypertrophy group, and the differences were statistically significant ( P 〈 0.05 ). Conclusion Speckle tracking imaging can assess left ventricular long- axis systolic and diastolic function in patients with essential hypertension.
