摘要
目的以脑缺血所致多器官功能障碍综合征(MODS)大鼠模型为研究对象,从组织形态学及基因表达(CD14 mRNA)角度,探讨针刺人中穴内关穴对MODS鼠肺组织的保护作用。方法采用阻断两侧颈总动脉建立脑缺血MODS大鼠模型。将100只大鼠随机分为正常对照组、假手术组、模型组、针刺组,其中模型组针刺组又分为1、6、24、72 h 4个时间段。采用动物呼吸机测定各组大鼠的呼吸频率;HE染色观察肺组织内细胞形态变化;原位杂交技术检测肺组织中CD14 mRNA基因表达。结果 (1)呼吸频率比较:针刺干预后,各针刺组呼吸频率评分较模型对照组均增加,24、72 h组的呼吸频率增加最为明显,与模型对照组、假手术组以及针刺1 h组均有统计学差异(P<0.05)。(2)HE染色镜下观察显示:模型组肺在脑缺血6 h后可见支气管黏膜假复层柱状上皮坏死脱落,黏膜下层有炎细胞浸润,24 h后肺组织结构消失,被大量炎性细胞代替,针刺组6 h肺泡壁明显增厚,水肿,内有大量单个核细胞浸润,针刺组24 h肺泡壁毛细血管狭长,扩张。(3)原位杂交技术检测。脑缺血并发MODS后6 h,大鼠肺组织CD14 mRNA表达呈升高趋势,24 h达到峰值。各针刺组大鼠肺组织CD14 mRNA表达明显低于相对应时相的模型组(P<0.05或P<0.01),24 h针刺组下降幅度最明显(P<0.01)。结论针刺人中、内关穴能改善脑缺血并发MODS鼠的呼吸功能,对肺组织具有一定的保护作用;降低CD14 mRNA表达,可能是针刺保护脑缺血并发MODS鼠肺组织损伤的机制之一。
Objective:To explore the protective effect of acupuncture at Renzhong(DU26) acupoint and Neiguan (PC6) acupoint for rat lungs under cerebral ischemia complicated by MODS.Methods:Model of cerebral ischemia MODS rats was established by blocking Rats' bilateral carotid artery.100 rats were randomly divided into normal control group,sham-operated group,model group,and acupuncture group.Meanwhile,both model and acupuncture groups were divided randomly into four time levels,which were 1 h,6 h,24 h,and 72 h.Breathing rates of rats were determined by using animal respirator.Lung cell-shape changes were observed with HE staining.And Gene expression of CD14 mRNA determined by situ hybridization.Results:(1)Comparison of breathing rates:after acupuncture intervention,breathing rates of all the acupuncture groups increased compared with model control groups,especially in 24 h and 72 h groups.There were statistical differences among model control group,sham-operated group and acupuncture 1h group (P < 0.05).(2) Microscopic observation under HE staining showed:bronchial mucosa pseudostratified columnar epithelial necrosis and inflammatory cell infiltration in submucosa were observed after stricken with cerebral ischemia for 6 hours.After 24 hours,lung tissue structure disappeared and was substituted by a big amount of inflammatory cells.Alveolar walls got thickened obviously with edema and a big amount of mononuclear cells infiltration in acupuncture groups.After 24 hours,the capillaries in alveolar walls got dilated in acupuncture 24 h group.(3) Test results of in situ hybridization:after cerebral ischemia complicated by MODS for 6 hours,the expression of CD14 mRNA increased with time,and reached the top at 24 h.The amount of CD14 mRNA expression was smaller than corresponding model group (P< 0.05 or P< 0.01),especially in 24 h acupuncture group which decreased obviously.Conclusion:The rats with cerebral ischemia complicated with MODS have a certain level of dyspnea,and acupuncture can improve respiratory function.Acupuncture at Renzhong (DU26) acupoint and Neiguan (PC6) acupoint has protective effects on lung tissues at certain level.3.Decreasing the expression of CD14 mRNA might be one of the mechanisms of protective effect of acupuncture for rat lung tissue under cerebral ischemia complicated by MODS.
