摘要
目的观察帕瑞昔布钠复合丙泊酚用于无痛宫腔镜检查术的麻醉效果及安全性。方法 60例ASAⅠ级或Ⅱ级行宫腔镜检查术患者,随机分为帕瑞昔布钠复合丙泊酚组（Ⅰ组）和丙泊酚组（Ⅱ组）,各30例。Ⅰ组静脉注射帕瑞昔布钠40mg（用0.9%生理盐水配置成4 mL）,20 min后缓慢推注丙泊酚1~2 mg/kg;Ⅱ组静脉注射0.9%生理盐水4 mL,20 min后缓慢静脉注射丙泊酚1.5~2.5 mg/kg,待患者入睡、睫毛反射消失、呼吸平稳后即可开始手术,必要时每次追加丙泊酚0.5~1mg/kg。观察患者麻醉前（T0）、睫毛反射消失时（T1）、宫腔操作时（T2）、检查结束时（T3）和苏醒时（T4）的SBP、HR、RR和SpO2变化;以及手术时间、术中丙泊酚总用量、苏醒时间,术中肢动、术后腹痛和麻醉后不良反应的发生情况。结果Ⅱ组患者在睫毛反射消失时的SBP较麻醉前有明显降低（P〈0.05）,在T1时Ⅱ组患者的SBP、RR明显较Ⅰ组低,Ⅱ组丙泊酚用量明显高于Ⅰ组（P〈0.05）。Ⅱ组苏醒时间长于Ⅰ组（P〈0.05）,Ⅱ组患者术中肢动、术后腹痛、麻醉后不良反应发生率明显高于Ⅰ组（P〈0.05）。结论帕瑞昔布钠复合丙泊酚可以安全用于无痛宫腔镜检查且镇痛效果满意。
[ Objective ] To evaluate the [ Methods ] Sixty ASA of Ⅰ or Ⅱ pati effect and safety of propofol with parecoxib in patients for painless hysteroscopy. ents undergoing analgesic were randomLy divided into two groups：group Ⅰ （30 cases ）was administrated propofol combined with parecoxib sodium. Group Ⅱ （30 cases ）was administrated propofol.SBP. HR.RR and SpO2 were observed of each group at five time point：before anesthesia（T0）.when loss of eyelash reflex（T1）. during operation（T2）.the end of operation（T3） and when awaking from operation（T4）.The time of operation.total amounts of propofol,time of wake-up were recorded. The incidence of pain after operation,the adverse reaction and complications of anesthesia were recorded. [ Resluts ] SBP at T1 time point was lower than that at To time point in group Ⅱ （P〈0.05）.
