胎盘位置及厚度影响早孕期经腹绒毛活检成功率的研究 被引量:3

摘要 目的探讨超声引导下经腹绒毛活检术中胎盘位置及厚度对穿刺成功率的影响。方法对绒毛活检术中不同胎盘位置及胎盘厚度的96例进行回顾性分析。结果 96例经腹绒毛活检术中90例一次穿刺成功,6例二次穿刺成功,一次成功率达93.8%。胎盘位于宫底部时经腹绒毛活检术一次成功率达100%,前壁胎盘时一次成功率为93.8%;后壁胎盘时一次成功率为85.0%。当胎盘厚度达0.9-1.0cm时一次成功率可达85%,二次成功率达100%。结论胎盘位置和厚度是影响经腹绒毛活检术成功率的重要因素,当肥盘厚度0.9cm-1.0cm时绒毛活检术更安全;胎盘附着在子宫前壁及宫底部和子宫后壁上部时,可顺利行经腹绒毛活检术,只有当子宫为后屈位且胎盘又附着于子宫后壁下段时不宜采用经腹绒毛活检术。 Objective: To evaluate the effects of placental thickness and position on the success rate of transabdominal chorionic villus sampling with ultrasonic guidance during the first trimester. Methods: A total of 96 pregnancies undergoing transabdominal chori- onic villus sampling were retrospectively analyzed. The success rate of sampling was evaluated by comparing placental thicknesses and positions. Results: Among these 96 cases, 90 were successfully drawn materials by transabdominal chorionic villus sampling in one time, 6 in twice. Chorionic villus were obtained in 93.8% cases via the first puncture. The success rate of the first puncture was 100% when the placentas located in uterine fundus, and the success rate was 93.8% with placentas covering the anterior uterine wall, 85% with placentas covering the posterior uterine wall. When the placental thicknesses exceeded 0. 9 - 1.0 cm, the success rate for transabdominal chorionic villus sampling was 85% with one puncture, all chorionic villus sampling being performed within two punc- tures. Conclusions: The placental thickness and position are very important for transabdominal chorionic villus sampling. The placen- tal thickness exceeding 0.9 cm makes chorionic villus sampling more safer. Transabdominal chorionic villus sampling can be performed when the placenta locate in uterine fundus or anterior uterine wall or the upper part of posterior uterine wall, and the menthod is not recommended for pregnant women with retroflexion of uterus and placenta covering the lower part of posterior uterine wall.
