髋部骨折合并下肢深静脉血栓形成134例术前干预及结果分析 被引量：8

摘要 目的 探讨髋部骨折合并下肢深静脉血栓形成（DVT）的术前治疗方法.方法 髋部骨折合并DVT患者134例,对58例周围型DVT允许延期手术者先行抗凝治疗2周,彩色多普勒声证实血栓机化后再进行骨科手术,术后低分肝素抗凝2周,之后改用华法林口服半年.对12例骨科手术不能延期者（小腿肌肉静脉丛及胫后静脉血栓）、11例股静脉及胭静脉血栓、53例中央型及混合型血栓放置下腔静脉临时滤器后手术,2周后取出滤器,滤器取出后改用华法林口服半年.结果 本组骨科手术皆顺利完成,术后随访6～26个月,期间无肺栓塞（PE）及严重DVT后遗症（如溃疡）出现.31例（23.1％）术后有下肢轻微水肿;15例DVT症状复发,经抗凝治疗后下肢肿胀缓解,继续门诊抗凝治疗;5例出现因抗凝治疗导致的出血并发症,经对症治疗后痊愈.结论 对于髋部骨折合并DVT的患者,术前依据DVT类型进行干预可预防PE等严重并发症发生.
