摘要
目的探讨联合检测高敏C-反应蛋白(hs-CRP)、抗环瓜氨酸肽抗体(抗CCP抗体)在类风湿关节炎(RA)诊断中的临床意义及应用价值。方法选择RA患者57例(RA组),对照组为排除与类风湿相关疾病健康体检者,用日立7180全自动生化分析仪、胶乳增强免疫比浊法检测hs-CRP和类风湿因子(RF),用罗氏E411全自动电化学发光免疫分析仪、电化学发光法检测抗CCP抗体,并对3项指标的检测结果进行分析比较。结果 RA组患者血清hs-CRP、RF、抗CCP抗体水平明显高于对照组,差异有统计学意义(P<0.05);RA组患者单项指标检测hs-CRP、RF、抗CCP抗体阳性率分别为66.7%、54.4%、73.7%,3项指标联合检测阳性率达91.2%,联合检测阳性率与单项指标检测阳性率比较,差异有统计学意义(P<0.05)。对照组单项指标检测hs-CRP、RF、抗CCP抗体阳性率分别为2.4%、4.9%、2.4%,3项指标联合检测阳性率为7.3%,联合检测阳性率与单项指标检测阳性率比较,差异无统计学意义(P>0.05)。结论血清hs-CRP、抗CCP抗体联合检测可显著提高对RA的诊断特异性,提高RA的检出率。
Objective To explore the clinical significance and the application value of high-sensitivity C-reactive protein(hs-CRP) and anti-cyclic citrullinated peptide(anti-CCP) antibody in diagnosing rheumatoid arthritis(RA).Methods 57 patients with RA were selected as RA group,while 44 healthy subject without relation to rheumatoid arthritis were chosen as control group.The Hitachi 7180 fully automatic biochemical analyser was used to detect hsCRP and rheumatoid factors(RF) by the latex enhanced immune turbidimetry and the Roche E411 fully automatic electrochemical luminescence analyser was used to detect anti-CCP antibody by the electrochemiluminescence method.The detection results of three indicators were comparatively ana lyzed.Results Serum hs-CRP,RF and anti-CCP antibody levels in the RA group were significantly higher than those in the con trol group with statistically significant difference(P0.05);the positive rate of hs-CRP,RF and anti-CCP antibody in the RA group was 66.7%,54.4% and 73.7% respectively,but the positive rate of the 3-index combination detection reached 91.2%,the difference in the positive rate between the combination detection and single index detection had statistical significance(P0.05).The positive rates of single hs-CRP,RF and anti-CCP antibody detection in the control group were 2.4%,4.9% and 2.4% respectively,the positive rate of the 3-index combination detection reached 7.3%,the difference in the positive rate between the combination detection and single index detection had statistical significance(P0.05).Conclusion The combination detection of serum hs-CRP and anti-CCP antibody can significantly improve the specificity to the diagnosis of RA and improve the detection rate of RA.
