油酸致RDS过程继发性肝损害机制探讨 被引量:8

摘要 目的 :探讨油酸肺损伤所致 RDS过程进行性氧分压下降与肝组织脂质过氧化、中分子物质和一氧化氮、内皮素以及肝功能动态变化之间的关系。方法 :随机取日本大耳白兔 36只 ,分为对照组和油酸 30 min、6 0 m in、90 m in、12 0 min组。检测 MABP、血气以及血浆和肝组织 GSH - Px、MDA、MMS、NO、ET和肝功能的动态变化 ,同时进行肝组织电镜观察。结果 :实验组 Pa O2 和 MABP随时间延长呈进行性下降。反映肝功能的血浆 AL T、AST、AL P含量在实验组分别从 6 0 m in、12 0 m in和 30 m in后逐渐增加。血浆 AL P和 AL T含量与 Pa O2 变化之间呈负相关关系 ;与肝组织 MDA含量变化之间呈正相关关系 ;与肝组织 MMS含量变化之间也呈正相关关系。实验组血浆和肝组织 GSH- Px含量随时间延长逐渐降低 ,而 MDA含量却随时间延长逐渐增加。肝组织 GSH - Px和 MDA含量变化与 Pa O2 下降之间分别呈正相关和负相关关系。肝组织 GSH- Px与 MDA含量变化之间呈负相关关系 ;肝组织NOS含量在油酸注入后 30 min时代偿性增加 ,而从 6 0 min后又逐渐降低。实验组血浆和肝组织 MMS含量随时间延长逐渐增高。肝组织 MMS含量与 Pa O2 变化之间呈负相关关系。结论 :油酸所致单纯肺损伤过程中可伴发肝功能和肝结构的损伤 ,此损伤变化与 Objective: It was designed to study the relation between the gradual reduction of PaO\-2 and the dynamic changes of lipid peroxidation, middle molecular substances(MMS), nitric oxide (NO), endotheliotoxin(ET) and liver function during oleicacid induced RDS. Methods: 36 rabbits was divided into 5 groups: OA 30 min, OA 60 min, OA 90 min , OA 120 min and control group. Developing changes of MABP, blood gas, and dynamic changes of GSH Px、MDA、MMS、NOS、ET of plasma and liver tissue homogenate were examined. At the same time, liver function was detected and ultramicrostructure of liver were observed under the electron microscope. Results: PaO\-2 and MABP of experimental group were decreased progressively. In experimental group , ALT, AST and ALP content reflecting liver function were increased gradually after 60 min,120 min and 30 min. There were negative correlation between the changes of ALP or ALT content and the changes of PaO\-2. There was a positive correlation between the changes of ALP or ALT content and the changes of MDA content of liver. There was a positive correlation between the changes of ALP or ALT content and changes of MMS content of liver. GSH Px content of plasma and liver were decreased as time went on, Nevertheless, MDA of plasma and liver were increased gradually. There was a positive correlation /or negative correlation separately between the changes of GSH Px /or MDA content of liver and decrease of PaO\-2. There was a negative correlation between the changes of GSH Px content and MDA content. NOS content of liver appeared compensatory increase at OA 30 min, but it was decreased gradually after OA 60 min. MMS content of plasma and liver of experimental group were increased gradually. There was a negative correlation between the changes of MMS of liver and the changes of PaO\-2. Conclusion: There is secondary hepatic damage during oleic acid induced lung injury (RDS) in the rabbit. The damage is associated with reduction of PaO\-2 and MABP, lipid peroxide, increase of MMS and losing balance between NOS and ET in the injury.
