摘要
【目的】观察清热解毒、活血化瘀中药治疗Ⅲ～Ⅳ期子宫内膜异位症不孕患者腹腔镜手术后的临床疗效。【方法】将经腹腔镜手术诊断为Ⅲ～Ⅳ期子宫内膜异位症不孕并行腹腔镜手术治疗的80例患者随机分为2组,治疗组50例采用清热解毒、活血化瘀中药治疗;对照组30例使用促性腺激素释放激素(GnRh-a)治疗。比较2组的怀孕率、痛经和巧囊的复发率,以及中医证候和痛经的临床疗效。【结果】(1)治疗后治疗组的总怀孕率显著高于对照组,差异有统计学意义(P<0.05);(2)治疗1年后治疗组的痛经复发率显著低于对照组,差异有统计学意义(P<0.05);(3)治疗1年后2组巧囊的复发率比较,差异无统计学意义(P>0.05);(4)治疗后2组患者的中医证候积分和痛经积分均显著下降,与治疗前比较差异均有统计学意义(P<0.01),且治疗组对中医证候积分和痛经积分的改善作用优于对照组(P<0.05或P<0.01)。【结论】对于Ⅲ～Ⅳ期子宫内膜异位症不孕患者,术后接受中医药辅助治疗,在提高怀孕率、降低复发率、减轻痛经程度上均有良好的效果。
Objective To observe the clinical effects of therapy of clearing heat and expelling toxicity, activating blood and removing stasis for the treatment of stage Ⅲ to Ⅳendometriosis infertility after laparoscopic operation. Methods The qualified 80 patients with stage Ⅲ to Ⅳendometriosis infertility after laparoscopic operation were randomized into treatment group and control group. The treatment group （N=50） was given Chinese herbal medicine with the actions of clearing heat and expelling toxicity, activating blood and removing stasis, and the control group （N=30） was given gonadotrophin-releasing hormone （GnRH） . Clinical outcomes of pregnancy rate, dysmenorrhea recurrence, and recurrence of endometrial cyst of ovary were observed in both groups. After treatment, the effects on relieving dysmenorrhea and traditional Chinese medical syndromes were also compared in both groups. Results After treatment, the treatment group had higher pregnancy rate than the control group, the difference being significant （P〈0.05） . The results of one-year observation showed that the recurrence rate of dysmenorrhea was lower in the treatment group than that in the control group （P〈0.05） . There was no significant difference in the recurrence rate of endometrial cyst of ovary one year after laparoscopic operation （P〉0.05） . After treatment, the scores of traditional Chinese medical syndromes and dysmenorrhea were both evidently improved in both groups （P〈0.01 compared with those before treatment）, and the improvement in the treatment group was superior to that in the control group （P〈0.05 or P〈0.01） . Conclusion For the patients with stage Ⅲ to Ⅳendometriosis infertility after laparoscopic operation, the application of Chinese herbal medicine with the actions of clearing heat and expelling toxicity, activating blood and removing stasis shows satisfactory effect on increasing pregnancy rate, reducing the recurrence of endometrial cyst of ovary and relieving dysmenorrhea.
