摘要
目的探讨腹腔镜手术联合温肾活血中药治疗子宫内膜异位症疗效及对血清Th1/Th2、氧化应激指标水平的影响。方法将170例子宫内膜异位症患者随机分为2组,对照组85例行单纯腹腔镜手术治疗,观察组85例在腹腔镜手术后加用温肾活血中药治疗3个月经周期,观察2组治疗前后中医证候积分及性激素、IL-2、IL-4、INF-γ、氧化应激指标及MCP-1水平变化情况,统计2组近期疗效及术后复发率。结果 2组治疗后痛经、经血紫暗、形寒肢冷、月经量少、性交痛、非经期下腹痛及神疲乏力积分均显著低于治疗前(P均<0.05),且观察组各项积分均显著低于对照组(P均<0.05);2组治疗后性激素、IL-2、IL-4、INF-γ、氧化应激指标及MCP-1水平均明显改善(P均<0.05),且观察组各项指标水平改善情况均显著优于对照组(P均<0.05)。观察组近期总有效率显著高于对照组(P<0.05),术后复发率显著低于对照组(P<0.05)。结论腹腔镜手术联合温肾活血中药治疗子宫内膜异位症可有效减轻临床症状,降低复发率,并有助于调节性激素、氧化应激指标及MCP-1水平。
Objective It is to investigate the clinical efficacy of laparoscopic surgery combined with Wenshen Huoxue prescription in the treatment of patients with endometriosis and influence on levels of Th1 / Th2 and oxidative stress. Methods 170 patients with endometriosis were chosen and randomly divided into 2 groups including control group（ 85 patients） which were treated with laparoscopic surgery alone and observation group（ 85 patients） treated with Wenshen Huoxue prescription on the basis of therapy in control group. The clinical effects for short-term,the recurrence rate after operation,the TCM symptoms scores,the levels of sex hormone,IL- 2,IL- 4,INF- γ,oxidative stress index and MCP- 1 before and after treatment of both groups were compared. Results The clinical effects for short-term of observation group was significantly higher than that in control group（ P 0. 05）. The recurrence rate after operation of observation group was significantly lower than that in control group（ P 0. 05）. The TCM symptoms scores after treatment of both groups were significantly lower than that before treatment（ P 0. 05）. The TCM symptoms scores after treatment of observation group were significantly lower than that in control group（ P 0. 05）. The levels of sex hormone,IL- 2,IL- 4,INF- γ,oxidative stress index and MCP- 1 after treatment of both groups were significantly better than before treatment（ P 0. 05）. The levels of sex hormone,IL- 2,IL- 4,INF- γ,oxidative stress index and MCP- 1 after treatment of observation group were significantly better than that in control group（ P 0. 05）. Conclusion Laparoscopic surgery combined with Wenshen Huoxue prescription in the treatment of endometriosis can efficiently relieve the clinical symptoms and signs,reduce the risk of recurrence rate and be helpful to regulate the levels of sex hormone,IL- 2,IL- 4,INF- γ,oxidative stress index and MCP- 1.
