抗MCV、抗RA33抗体、RF联合检测在类风湿关节炎诊断中的应用 被引量:1

摘要 目的:探讨抗突变型瓜氨酸波形蛋白(anti-MCV)抗体、抗RA33抗体和类风湿因子(RF)联合检测在类风湿关节炎(RA)中的临床应用价值。方法:对95例RA患者和50例其他自身免疫疾病患者及50名正常对照者检测抗MCV、抗RA33抗体和RF,观察各指标及3项联合检测对RA的诊断价值。结果:单指标检测抗MCV抗体对RA的敏感性最高;抗RA33抗体对RA的特异性最高。三项指标中任一指标阳性即作为阳性的敏感性最高(95.12%);而三指标均为阳性才视为阳性的敏感性略低于RF,但特异性可达99.00%。结论:抗MCV、抗RA33抗体、RF是RA较为特异的指标,3项指标联合检测可以把阳性患者的误诊率降到最低,有助于对RA的早期诊断、治疗和病情预后估计。 Objective:To investigate the clinical value of joint detection of anti -mutant citrullinated vimentin ( anti - MCV) antibody, anti - RA33 antibodies and rheumatoid factor (RF) in rheumatoid arthritis (RA). Meth- ods: Ninety - five RA patients, 50 patients with other autoimmune disease and 50 normal controls were involved for detection of anti - MCV, anti - RA33 antibodies and RF, and the diagnostic value of each indicator and joint detec- tion for RA was observed. Results: In single indicator detection, anti - MCV antibodies showed the highest sensitiv- ity for RA, while anti - RA33 showed the highest specificity for RA. Any of the three indicators showing positive was regarded as the one with highest sensitivity(95.12% ), while three indicators all positive was regarded as sensi- tivity lower than RF, but the specificity can reach 99.00%. Conclusion : The anti - MCV, anti - RA33 antibodies, RF are the specific indicators of RA. Joint detection of three indicators can minimize the misdiagnosis rate of patients with positive, contribute to the early diagnosis, treatment and prognosis estimation of RA.
