摘要
目的:研究BRCA1和Ki-67蛋白在新疆维吾尔族及汉族三阴性乳腺癌组织中的表达及临床意义。方法:采用免疫组化方法检测115例维吾尔族三阴性乳腺癌、35例乳腺纤维腺瘤及120例汉族三阴性乳腺癌组织中BRCA1和Ki-67蛋白的表达,并结合临床病理因素进行相关分析。结果:BRCA1在乳腺癌组织中阳性表达率39.6%(93/235),在乳腺纤维腺瘤组织中阳性表达率97.1%(34/35);Ki-67在乳腺癌组织中的阳性表达率81.3%(191/235),高于乳腺纤维腺瘤组的20.0%(7/35),差异有统计学意义,P<0.05。新疆维吾尔族、汉族两民族间BRCA1表达分别为73.0%(84/115)和48.3%(58/120),差异有统计学意义,χ2=14.99,P<0.05。BRCA1蛋白阳性表达率随乳腺癌组织学分级的增高而降低,在淋巴结转移组阳性表达率低于淋巴结无转移组;在年龄≤40岁组阳性表达率低于年龄>40岁组,差异均有统计学意义,P<0.05。Ki-67在乳腺癌组织中表达与患者年龄、组织学分级和淋巴结转移有关,P<0.05;而与肿瘤大小及民族因素无关,P>0.05。BRCA1和Ki-67蛋白的表达呈负相关,r=-0.281,P<0.05。结论:新疆维吾尔族、汉族三阴性乳腺癌组织中BRCA1和Ki-67蛋白表达存在差异,BRCA1失表达可能与早发性三阴性乳腺癌有关,并与Ki-67高表达相关。提示三阴性乳腺癌肿瘤增殖快恶性程度较高。
OBJECTIVE:To study the expression of BRCA1 ,Ki-67 and the clinical significance in Uygur and Han na tionality with triple-negative breast cancer. METHODS: Immunohistochemical method was used to detect the BRCA1 and Ki-67 protein in this study,which including 115 Uygur patients and 120 Han patients with triple-negative breast cancer, and 35 benign mammary fibroma patients. The relationship and clinical pathological features were analysed. RESULTS: The expression of BRCA1 protein in breast cancer tissue was (39.6%, 93/235), while in mammary fibroma tissue was (97.1 %,34/35) ;The expression of Ki-67 protein in breast cancer tissue was higher than that in mammary fibroma tissue (81.3 %vs 20.0%, P〈O. 05). The expression of ERCA1 protein reduced with the histological grade increased, the ex pression of BRCA1 protein in lymph node metastasis group was less than that without lymph node metastasis groups in age ≤40 group,the positive expression rate was less than age 〉40 group (P〈0.05) ; There was also statistical difference between Han and Uygur patients with breast cancer in expression of BRCA1 (73.0%vs 48.3 %,X2= 14.99, P〈0.05).There were also statistical differences (P〈0.05) in the expression of Ki-67 with age,histological grade and lymph node metastasis,and no statistical difference with tumor size and ethnic factors (P〈0.05). Negative correlation was found be tween BRCA1 and Ki-67 expression in breast cancer (r= 0. 281, P〈0.05). CONCLUSION: There is statistical differ ence in the expression of BRCA1 and Ki-67 between the IJygur and Han nationality, and the aberration expression of BRCA1 protein may be related to the early onset breast cancer,suggesting poor prognosis and high malignancy of triple negative breast cancer.
