摘要
目的研究直肠癌手术质量控制指标,建立及时、客观和合理的直肠癌手术质量评价体系。方法对2009年11月至2011年12月收治的230例直肠癌患者作为研究对象,分析2位主诊医师团队的手术时间、术中出血、术后通气时间、术后吻合口漏、术后肛门功能和局部复发率,并进行统计学分析。结果总手术时间平均(146±27)min,总术中出血量(105±4)ml,术后总平均通气时间(3.78±1.31)d,吻合口漏发生率1.3%,86.4%(102/118)患者控便功能基本正常(以得分0～5分患者统计),本组7例患者发生局部复发,局部复发率3.0%。2位主诊医师团队在手术时间、术中出血、手术后通气时间、手术后吻合口漏、手术肛门功能和局部复发差异无统计学意义。结论手术时间、术中出血、术后通气时间、吻合口漏、肛门功能和局部复发等6个指标作为直肠癌手术质量控制的理想指标,合格的直肠癌手术医师至关重要。
Objective To investigate the correlated indexes for evaluating the operative quality of rectal cancer,and to establish a timely,objective and reasonable evaluation system.Methods The clinical data of 230 patients with rectal cancer treated from November 2009 to December 2011 were analyzed retrospectively in terms of operative duration,blood loss,postoperative flatus,anastomotic leakage,anal function and local recurrence.Results The operative duration was（146 ± 27） min,and the mean blood loss was（105 ± 4） ml.The mean postoperative flatus was（3.78 ± 1.31） days and the incidence of anastomotic leakage was 1.3%.86.4%（102 /118） of the patients showed fair anal function（Wexner Score ranged from 0-5） and 7 patients（3.0%） had local recurrence.Conclusions The operative duration,blood loss,postoperative flatus,anastomotic leakage,anal function and local recurrence were ideal factors for evaluating the operative quality of rectal cancer.
