摘要
探讨精神障碍诊断量表 (DSMD)七轴诊断系统对精神分裂症的适用性。方法 :选取全国 2 3家协作单位应用DSMD测试并按CCMD - 2诊断的 17种病类 72 6例患者的资料 ,由三位经DSD及CCMD - 2R、ICD - 10等诊断标准训练的精神科医师进行再诊断。对其中应用CCMD -2R诊断的 382例精神分裂症患者的七轴诊断结果进行分析。结果 :DSMD的七轴诊断系统对全面分析精神分裂症患者的总体临床状况是可行的。它既使轴 1主要与次要诊断按急需原则与就重原则等级排列一目了然 ,也明确了轴 2人格特征、人格障碍或人格改变及轴 3躯体障碍的诊断。并评估了轴 4生物心理社会因素的刺激强度及种类 ,轴 5社会功能在疾病最重时损害 ,目前社会功能及病前 2年的最佳水平 ;轴 6现状总评对病情做出总体评估。同时在轴 7说明了精神障碍与其他相关问题间的关系。结论 :DSMD的七轴诊断系统适用于精神分裂症的诊断。
To investigate the applicability of the seven-axial diagnostic system for schizophrenic patients. Methods: Based on the DSMD Diagnostic Scale for Mental Disorders (DSMD), 726 patients with 17 established diagnoses of mental disorders were re-assessed independently by three psychiatrists, using a double-blind method. 382 of these patients met the criteria of CCMD-2R for schizophrenia were analyzed. Results: In addition to Axis-1 diagnoses of mental disorders, the seven-axis system provided clear and useful information on personality functioning (Axis-2), physical disorders (Axis-3), types and degrees of biopsychosocial stressors (Axis-4), current and pre-morbid social functioning (Axis-5), current global condition (Axis-6), and relationship with other relevant conditions (Axis-7). Conclusion: The results supported the use of the seven-axis diagnostic system of DSMD for schizophrenic patients.
