摘要
目的了解珠海市人民医院重症监护科营养支持药物的应用情况,探讨肠内营养(EN)、肠外营养(PN)、肠内肠外营养(EN+PN)的临床治疗效果。方法采用药物利用分析方法对珠海市人民医院2010-09～2011-08重症监护科营养药物的用药频度(DDDs)、药物利用指数(DUI)进行分析;并选取此期间该科患者119例,分别按EN组32例、EN+PN组55例、PN组32例进行回顾性调查,考察其营养支持前后血红蛋白(Hb)、血清白蛋白(sALB)的变化,进行分析比较。结果 DDDs排序前3位的药品分别是人血白蛋白(5 g)、肠内营养混悬液(能全力)、5%复方氨基酸注射液(18AA)。3组患者的Hb及sALB均有不同程度降低,PN组降低最多。PN组与EN组、PN组与EN+PN组间比较有显著性差异(P<0.01),EN组与EN+PN组间比较则没有显著性差异(P>0.05)。各组用药时机基本与指南相符。结论本院重症监护科营养支持药物的应用基本合理。EN或EN+PN的营养治疗效果优于PN,重症监护患者应积极采用EN。
Objective To find out the application situation of nutrition support drugs in intensive care unit （ICU） in People＇ s Hospital of Zhuhai, to investigate the clinical effect of enteral nutrition （ EN）, parenteral nutrition （PN） and enteral parenteral nutrition （ EN ＋ PN）. Methods The method of the application of the nutrition support drugs was adopted to analyze its situation in ICU in the hospital during 2010 -09 ~ 2011 -08 with respect to the frequency of use （DDDs） and drug utilization index （DUI）. Retrospective study of nutrition of inpatient was conducted in ICU, a total of 119 patients were divided into EN, PN or EN ＋ PN group according the nutritional route, and each group was 32, 32 and 55 cases. The changes in the number of hemoglobin （Hb） and serum albumin （sALB） were used as comparison index. Results The frist 3 kinds of drugs in DDDs were： human albumin, compound amino acid injection （18AA）, enteral nutritional suspension （TPF）. The level of Hb and sALB were descend in 3 groups and there was significant difference of descend between PN and EN, either PN and EN ＋ PN （P 〈 0.01 ）. But no significant difference of descend between EN and EN ＋ PN （ P 〉 0.05 ）. The time for starting the application of nutrition support drugs were basically matched with guidelines. Conclusion The application of nutritional support is rather appropriate in the ICU, EN or EN ＋ PN could reduce the descend of Hb and sALB significantly than PN.
