The status of malnutrition among the children aged 2~5 years old and the effect factors

摘要 目的:了解安徽省某市城区2~5岁在园儿童营养状况,分析相关影响因素,为改善该地区儿童营养状况提供依据。方法:采用分层整群抽样的方法,随机抽取4所公立幼儿园和8所私立幼儿园,对每所幼儿园在园儿童进行一般情况调查、体格发育测量和口腔检查,并采末梢静脉血检测血红蛋白。结果:共调查2 993例儿童,男1 424例,女1 569例,平均月龄为(51.23±11.73)个月,公立幼儿园儿童占64.0%,低体重、生长发育迟缓和消瘦检出率分别为4.2%、4.1%和3.7%,儿童营养不良检出率为7.9%,儿童贫血检出率为33.0%,龋齿检出率为47.4%。单因素分析显示,男童、贫血、私立幼儿园儿童营养不良检出率较高,月龄越大儿童营养不良检出率越低,患龋齿儿童营养不良检出率较低。多因素Logistic回归分析结果显示,女童〔0.525(0.397~0.693)〕、月龄高〔0.976(0.964~0.987)〕和牙齿数〔0.783(0.624~0.983)〕多为儿童营养不良的保护因素,私立幼儿园〔2.522(0.397~3.336)〕为儿童营养不良的危险因素。结论:该地区私立幼儿园和低龄儿童营养不良检出率较高,应针对私立幼儿园和低龄儿童实施针对性干预,控制营养不良儿童的发生。 Objective:To understand the nutritional status of children aged 2~5 years old from kindergartens of the urban area of one city,analyze the related effect factors,provide a basis for improving the nutritional status of children in the local area.Methods:Stratified cluster sampling method was used to abstract 4 public kindergartens and 8 private kindergartens from the urban area of one city randomly,the general conditions of all the children in kindergartens were surveyed,physical development examination and oral examination were carried out,and their peripheral blood samples were obtained to detect hemoglobin level.Results:2 993 children were investigated,including 1 424 boys and 1 569 girls,the average age was(51.23±11.73) months,the proportion of children in public kindergartens was 64.0%,the detection rates of low birth weight,slow physical growth,emaciation,malnutrition,anemia and decayed tooth were 4.2%,4.1%,3.7%,7.9%,33.0% and 47.4%,respectively.Single factor analysis showed that the detection rates of malnutrition in boys,the children with anemia and the children in private kindergartens were relatively high;the older the children was,the lower the detection rate of malnutrition was;the children with decayed tooth had low detection rate of malnutrition.Multivariable logistic regression analysis showed that the protective factors of malnutrition included girls(0.525(0.397~0.693)),high age(0.976(0.964~0.987)) and the number of decayed tooth(0.783(0.624~0.983)),the risk factor of malnutrition was private kindergartens(2.522(0.397~3.336)).Conclusion:In the area,the detection rates of malnutrition among the children in private kindergartens and the children with low age are high,intervention measures targeting to the children in private kindergartens and the children with low age should be carried out to control the occurrence of malnutrition among children.
