摘要
目的观察硫辛酸(alpha-1ipoic acid,ALA)和甲钴铵(Mecobalamin)分别治疗糖尿病周围神经病变(diabetic peripheral neuropathy,DPN)的临床疗效。方法将46例2型糖尿病合并周围神经病变患者随机分成治疗组和对照组各23例,在控制血糖的基础上,治疗组予以ALA注射液600mg加入生理盐水250mL中静脉滴注,每日1次,疗程4周;对照组每日口服甲钴铵1mg,每天三次治疗,疗程4周,观察治疗后临床症状改善情况,并测定治疗前后正中神经和腓总神经的运动传导速度和感觉传导速度。结果治疗4周后,治疗组总有效率91.30%(21/23),对照组总有效率73.91%(17/23)。治疗组临床疗效较对照组明显改善(P<0.05)。且两组的神经传导速度均提高,治疗组神经传导速度提高更明显(P<0.05)。结论 ALA较口服甲钴铵更能明显改善DPN的临床症状及神经传导速度,且无不良反应,值得临床推广使用。
Objective To observe the therapeutic efficacy between treating DPN by alpha-1ipoic acid or Mecobalamin.Methods 46 cases of T2DM were randomly assigned into treatment group and control group,with 23 cases in each group.Basing on controlling the level of Blood glucose,patients in the treatment group were treated by alpha-1ipoic acid（600mg） which is added into 250ml physiological saline for infusion solution,once a day and continuously for 4 weeks;patients in the control group were treated by taking Mecobalamin Tablets（1mg）,three times a day and continuously for 4 weeks.And then observe both the therapeutic efficacy and the conduction speed of median nerve and peroneal nerve.Results After 4 weeks treatment,the total effective rate was 91.30% in the treatment group and 73.91% in the control group.The therapeutic efficacy in the treatment group is superior to that in the control group（P0.05）.The nerval conduction speed in both groups is improved,and the improvement of nerval conduction speed in the treatment group is superior to that in the control group（P0.05）.Conclusion Alpha-1ipoic acid treatment is much more effective then Mecobalamin treatment in improving the nerval conduction speed which caused by DPN,and less side effect.It is worth to extend.
