摘要
目的探讨256层螺旋CT冠状动脉血管成像对成人冠状动脉起源异常的诊断价值。方法对中日友好医院2010年2月-2011年3月经256层螺旋CT进行冠状动脉血管成像的患者资料进行回顾性分析、总结,以发现成人冠状动脉起源异常的病例。心率<65次/m in的患者采用前瞻性心电门控扫描;心率在65～93次/m in之间的患者采用回顾性心电门控扫描。结果 1176例完成256层螺旋CT冠状动脉血管成像的患者共检出冠状动脉起源异常19例,检出率为1.62%,其中男性12例,女性7例,患者均有不同程度胸闷、胸痛等症状。检出类型包括右冠状动脉起源异常11例,左冠状动脉起源异常3例,左、右冠状动脉均起源异常1例,回旋支起源异常3例和右冠状动脉缺如1例。结论 256层螺旋CT诊断成人冠状动脉起源异常,是一种无创、安全和有效的方法。
Objective To explore the value of 256 multislice computed tomography（MSCT） in the evaluation of coronary artery origin anomalies in adults.Methods Chinese adult patients with coronary artery CT angiography（CTA） between February,2010 and March 2011 from China-Japan Friendship Hospital were analyzed.Patients with heart rate65 bpm underwent prospective ECG-trigger scan.Patients with heart rate between 65-93 bpm underwent retrospective ECG-trigger scan.Results Nineteen out of the 1176 patients underwent CTA using 256 MSCT were found to have anomalous origin of the coronary artery（1.62%） with 12 male and 7 female.All the patients had chest tightness and pain.Coronary artery origin anomalies included anomalous origin of the right coronary artery（RCA） in 11 patients,the left coronary artery（LCA） in 3 patients,both RCA and LCA in 1 patients,the anomalous origin of the left circumflex artery（LCX） in 3 patient and the congenital absence of right coronary artery（RCA） in 1 patient.Conclusion Noninvasive 256 multislice computed tomography can effectively detect the anomalous origin of the coronary artery with high resolution and no restriction of heart rate.
