摘要
目的:探讨256层螺旋CT冠状动脉成像心电编辑功能对于改善扫描过程中心律失常或心电图部分信号异常中冠状动脉成像质量的作用。方法:收集14例在检查过程中出现心律失常或同步记录的心电图信号异常的256层螺旋CT冠状动脉成像的资料,使用心电编辑功能,对心电图进行编辑,比较编辑前后重建的冠状动脉成像质量。结果:14例中,1例为房颤,6例为室性期前收缩,3例为不明原因心电图缺失,4例为识别异常R波。心电编辑前,由于心律失常,造成重建的冠状动脉图像模糊、缺失、阶梯状伪影,可评估的冠状动脉节段为54.7%;心电编辑后图像质量改善,可评估的冠状动脉节段达到86.6%(P<0.01)。结论:在256层螺旋CT冠状动脉成像检查过程中出现的心律失常或者不明原因出现的心电信号异常,使用心电图编辑功能,能显著提高冠状动脉成像质量。
Objective:To investigate the value of ECG editing in improving the image quality of 256-slice spiral CT coronary angiography in the presence of arrhythmia or unidentify synchronously record ECG error.Methods:The ECG-gated 256-slice spiral CT coronary angiographies of patients with arrhythmia or unidentifiable ECG mechanic error occurred during examination were retrospectively analyzed.The coronary angiographies were obtained with original synchronously recorded ECG and with the ECG after edition with the builtin ECG editing software.The image quality of reconstructed coronary before and after the ECG edit was assessed using evaluable vascular number.Results:Fourteen out of 335 patients with 256-slice spiral CT coronary angiography had abnormal ECG during examination,including 1 atrial fibrillation,6 premature ventricular contractions,3 partially unidentifiable ECG missing,and 4 falsely recognized R wave.The arrhythmia and ECG partially missing resulted in blurring,segmental missing,significant ladder-like artifacts in reconstructed coronary arteries.54.7%(110/201)coronary segments were assessable for diagnosis with original ECG.After ECG editing,the image quality of coronary segments was obviously improved with an increase of the assessable segments to 86.6%(174/201).There was significantly statistical difference between them(P0.01).Conclusion:The ECG editing could significantly improve the image quality of MSCT coronary angiography in the presence of arrhythmia during the examination or unknown mechanic ECG failure.
