摘要
目的探讨2型糖尿病患者血清Fractalkine(FKN)水平与血糖、血脂的关系。方法 2型糖尿病患者130例,根据尿白蛋白排泄率(UAER)分为以下三组:正常白蛋白尿组(NA)45例(UAER〈20μg.min-1);微量白蛋白尿组(MA)43例(UAER 20~200μg.min-1);大量白蛋白尿组(CP)42例(UAER〉200μg.min-1)。另选健康体检者40例为对照组(NC)。采用酶联免疫吸附法测定三组患者及对照组的血清FKN浓度,并分析血清FKN水平与空腹血糖(FPG)、餐后2小时血糖(2 h PG)、糖化血红蛋白(HbA1c)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)以及血压的关系。结果随着UAER的升高,血清FKN浓度逐渐升高,各组间差异均有统计学意义(P〈0.05)。相关分析显示,FKN与UAER、FPG、2 h PG、HbA1c、TC、LDL-C呈正相关,与HDL-C呈负相关。以FKN为应变量行多元逐步回归分析显示,FKN与UAER、LDL-C呈正相关。结论 FKN与糖尿病肾病的发生发展关系密切;2型糖尿病患者血清FKN水平与血糖和血脂代谢紊乱有关。
Objective To explore the relationship of serum fractalkine(FKN)level with blood glucose and lipid for the patients with type 2 diabetic nephropathy(2-DN).Methods 130 cases of 2-DN were divided into three groups according to urinary albumin ejection rate(UAER),including an anormoalbuminuria(NA)group(45 cases,UAER<20 μg·min-1),a microalbuminuria(MA)group(43 cases,UAER in the range from 20 to 200 μg·min-1)and a high-grad proteinuria(CP)group(42 cases,UAER>200 μg·min-1).Additionally,40 people for healthy examination were selected as normal control(NC)group.Enzyme-linked immunosorbent(ELISA)was used to detect serum FKN concentration of the patients in three groups and the people in control group.Besides,the relationship of serum FKN level with fasting plasma glucose(FPG),2 hours postoprandial blood glucose(2 h PG),glycosylated hemoglobin(HbA1c),total cholesterol(TC),triglyceride(TG),high density lipid-cholesterol(HDL-C),low density lipid-cholesterol(LDL-C)and blood pressure(BP)was analyzed.Results With UAER increasing,serum FKN concentration was up-regulated gradually,and the differences among groups were significant in statistics(P<0.05).The correlation analysis indicated that the positive correlation of FKN presented with UAER,FPG,2 h PG,HbA1c,TC and LDL-C,and the negative correlation was displayed between FKN and HDL-C.By taking FKN as strain,the multiple stepwise regression analysis showed that FKN was positively correlative with UAER and LDL-C.Conclusion FKN is closely relevant with the occurrence and development of DN.Serum FKN level for the patients with 2-DN is relevant with the metabolic disturbance of blood glucose and lipid.
