摘要
目的监控双极植入式微波辐射在治疗肝癌中的三维热场,探索计算机模拟预测温度场的方法;研究双导热场的温升规律,为临床一次性完全覆盖凝固较大肝肿瘤提供有效方法。方法在单导电极微波热场的基础上,采用计算机模拟,得到三种不同功率及作用时间下(40W30min,50W10min,60W300s)双针摆放的最佳距离。在离体猪肝对156个点次进行了双电极微波辐射的测温,以对计算机模拟结果进行检验、校正。结果在计算机给定两针最佳摆放距离情况下,离体猪肝温升曲线及凝固范围实测与模拟值均有较好的吻合性。40W30min两微波天线距离2.2cm时,凝固范围为6.0cm×6.0cm;50W10min两微波天线距离1.6cm时,凝固范围为4.7cm×5.3cm;60W300s两微波天线距离1.2cm时,凝固范围为3.9cm×4.9cm。结论双微波电极同时植入,可以形成较大的凝固性坏死范围,为临床一次原位整体灭活较大肝肿瘤提供了技术条件。根据热场叠加的基本理论及实验模拟研究结果,为今后计算机模拟组合复杂热场,超声引导凝固较复杂形态肝肿瘤奠定了基础。
Objective\ To observe three dimensional thermal distribution and temperature rise pattern of microwave radiation induced through double implanted microwave electrodes in coagulating liver cancer, to develop the calculating method with computer simulation in order to complete coagulate larger tumor at one stroke. Methods\ Based on thermal distribution of single electrode, the proper distance between two electrodes was determined by computer simulation at three kinds of power output (40 W 30 min, 50 W 10 min and 60 W 300 s). Above result was examined and correct through monitoring the temperature at 156 spots/times in 156 pork liver's tissue in vitro. Results\ Temperatures rise curve and coagulated area obtained from microwave radiation in the pork liver induced through the double electrodes coincided with value of computer simulation. Coagulated area was 6.0 cm×6.0 cm when the distance of 2.2 cm between two microwave electrodes at power output of 40 W 30 min. Coagulated area was 4.7 cm×5.3 cm when the distance of 1.6 cm between two microwave electrodes at power output of 50 W 10 min. Coagulated area was 3.9 cm×4.9 cm when the distance of 1.2 cm between two microwave electrodes at power output of 60 W 300 s. Conclusions\ A larger coagulate necrosis area was induced by inserting two microwave electrods simultaneously. It provides a technical basis for eliminate the liver cancer in situ at one stroke. Base on the interfere theory of thermal field and results of computer simulation, it is possible to simulate the complex thermal field with computer and to coagulate the irregular liver tumor in the feature.
