摘要
目的观察左旋氨氯地平联合厄贝沙坦治疗原发性高血压合并左心室肥厚的疗效、不良反应及对左心室肥厚的影响。方法 42例原发性高血压合并左心室肥厚患者,随机分为对照组和治疗组各21例,对照组左旋氨氯地平2.5mg晨服1次/d;治疗组在对照组基础上加用厄贝沙坦150mg晨服1次/d。两组疗程均为24周,观察治疗前后的血压变化、心肌肥厚状况、不良反应。结果两组患者总有效率间差异无统计学意义,P>0.05;治疗组中21例原发性高血压合并左心室肥厚患者的左心室舒张末期内径、室间隔厚度和左心室后壁厚度均值较治疗前缩小,差异有统计学意义,P<0.05;而对照组中21例心肌肥厚患者的相应指标间差异无统计学意义,P>0.05。两组的不良反应有头晕、头痛、面红或干咳,患者均能耐受。结论左旋氨氯地平联合厄贝沙坦治疗原发性高血压合并左心室肥厚疗效确切,且可以逆转心肌肥厚。
Objective To observe the efficacy,adverse reactions and the inpact on left ventricular hypertrophy of L-amlodipine combined with irbesartan treatment of essential hypertension and left ventricular hypertrophy.Methods 42 patients with essential hypertension and left ventricular hypertrophy were randomly divided into control group （21 patients） and treatment group （21 patients）.The patients in control group were given L-amlodipine 2.5mg morning service（day 1）,and the patients in treatment group were treated with L-amlodipine 2.5mg and irbesartan 150mg morning service（day 1）.Both groups were treated for 24 weeks.The changes in blood pressure,myocardial hypertrophy status and adverse reactions were observed before and after treatment.Results The total effective rate between the two groups were no statistically significant difference （P0.05）.21 cases with essential hypertension and left ventricular hypertrophy in the treatment group,the mean of left ventricular end diastolic diameter,interventricular septum thickness and left ventricular posterior wall thickness was narrowwed compared with pre-treatment,the difference was statistically significant（P0.05）;In the control group,the difference between the corresponding indicators in 21 cases with cardiac hypertrophy was not statistical significance （P0.05）. The adverse events in both groups were dizziness,headache,face red or mild dry cough,and the patients can tolerate.Conclusion There is curative effect in L-amlodipine combined with irbesartan treatment of essential hypertension and left ventricular hypertrophy,which can reverse the cardiac hypertrophy.
