摘要
目的探讨直接法多层螺旋CT静脉造影(MSCTV)在下肢深静脉血栓中的临床应用价值。方法对临床高度怀疑下肢深静脉血栓疾病的27例患者采用不同注射速率进行直接法MSCTV,所有采集的原始数据导入Wizad高级后处理工作站,经最大密度投影(MIP)、多平面重组(MPR)和容积再现法(VR)重建,分阶段对比分析图像质量,并与24例行DSA下肢深静脉造影的门诊患者进行分阶段静脉显示能力对比分析。结果①所有受检者血管重建显示良好,24例可显示不同部位、不同程度深静脉血栓形成,表现原始轴位图上血管腔内的充盈缺损,重建图像上显示管腔狭窄、中断或变细,明显浅静脉、侧支静脉扩张、纡曲。②采用不同注射速率进行直接法MSCTV在下肢深静脉血栓图像质量显示方面差异具有显著性,直接法MSCTV与DSA下肢静脉造影对腓肠静脉、腘静脉和股静脉的显示能力相同(χ2=1.021,P=0.312),直接法MSCTV显示髂静脉的能力优于DSA下肢静脉造影(χ2=4.364,P=0.037)。结论直接法MSCTV在下肢深静脉血栓中具有重要应用价值,显示不同节段静脉需要不同的注射速率。
【Objective】 To evaluate the application value of direct multi-slice computed tomography venography (MSCTV) in lower extremity deep vein thrombosis.【Methods】 27 patients highly suspected deep vein thrombosis by different injection rate of contrast agent underwent direct MSCTV.The original data were put into Wizad workstation and reconstructed with maximum intensity projection (MIP),multiplanar reformation (MPR),volume representation (VR),and the images quality were analyzed by stages.The displaying ability of direct MSCTV in lower extremity vein segment was compared with that of digital subtraction angiography (DSA) venography in 24 patients.【Results】Direct MSCTV shows reconstruction images clearly in all patients,24 cases display different parts and different degrees of thrombosis.The original axial images show filling defect of vascular,reconstruction image stenosis,segmental interruption or thinning in vascular.The superficial veins compensatory expansion,deep veins transport branch increased significantly.The image quality scores of 27 patients underwent direct MSCTV by different injection rate were statistically different.The displaying ability of direct MSCTV and DSA venography in calf vein,popliteal vein and femoral vein was not statistically different (χ2=1.021,P =0.312).For iliac vein,the displaying ability of direct MSCTV outstripped that of DSA venography (χ2=4.364,P =0.037).【Conclusion】 Direct MSCTV of lower extremity deep vein thrombosis has an important value,and the injection rate are different in displaying ability on the segmental veins.
